We've been ordering forms from this company for about 18 years. They've always delivered my order correctly and faster than quoted. The forms they produce are the best forms we've ever used. Our patients have actually mentioned how easy the forms made the intake process. The website could use some help but my front desk girl has no problem making it work. I would not consider ordering forms from anyone else.

Terrible Service!!! **DO NOT ORDER** This company got my order incorrect after numerous attempts to place an online order with no success due to their poorly functioning website and having to call in to place it. Received wrong items but a correct invoice. They never bothered to double-check my order prior to sending it out. They then insisted they could not expedite the replacement order, the gentlemen who answers the phone- Maurice is RUDE. He hung up in my face and told me to go somewhere else. This is how they treat their customers when they make a mistake filling orders. Obviously, they don't need the business.
