        Founded in 1955, PHASE, also known as Personal Health/Abuse Services & Education, is a health and human services agency that helps adults and their children to deal with problems arising from substance abuse and domestic violence. It is a nonprofit organization that offers outpatient substance abuse treatment. PHASE is licensed and partially funded by the Illinois DHS Division of Alcoholism and Substance Abuse. Additionally, it is a Medicaid-certified organization. PHASE is located in Rockford, Ill.
