Melton-Rodgers et al compared the aerobic effects of land-based biking versus water-based running for subjects with a diagnosis of class II or III adult-onset rheumatoid arthritis.2 There were no significant differences between the two training environments for the following factors: peak VO2, maximum heart rate, perceived exertion (RPE) at 60 percent peak VO2, or pain. Peak minute ventilation on the bike was 26 percent higher and peak tidal volume was 48 percent greater than that achieved during water running (significantly different). Conversely, peak RPE was 7 percent higher and peak respiration rate was 22 percent greater in the water than during land-based bicycling (significantly different). The authors concluded that it was possible to achieve training effects during water running for the population and parameters studied.

Fibromyalgia

Internal Medicine
Stenstrom et al examined the effects of a 1x/week aquatic exercise class for 30 subjects with class II RA over a 4 year span. [5] After four years of training 1x/week, the following significant improvements were evident in the training group: grip strength, frequency of exercise and frequency of hospitalization in the Department of Internal Medicine. Additionally, the control group attended supplemental physiotherapy visits more often than the training group (34 visits and 21 visits, respectively). At the two-year follow-up, the training group remained significantly more active and more likely to exercise than the control group. A majority of the training group continued to independently perform intensive, dynamic water training even though this was not offered as a service through the study. All but one subject in the training group rated the long-term benefits of the 1x/week training as "important" or "rather important."

Historically, FMS has been a difficult disorder to treat in the rehabilitation profession. Success in the treatment of FMS is obtained only through a comprehensive and multidisciplinary approach. Dr. Irene Metro, MD is a board certified internal medicine physician and one of the area’s leaders in the treatment of FMS. She is the owner of Plymouth Intergrated Medicine (PIM). Many large therapy centers do not provide warm water pool therapy. They claim that exercising in water does not provide carry over for people to function in a gravity environment. This is simply not true. Water provides the ideal medium for exercise allowing maximal muscle and joint function. The buoyancy of water decreases joint compression, limits mechanical stresses on soft tissues, and counters the effect of gravity on venous pooling in the limbs leading to increased blood volume centralization. The viscosity of water along with hydrostatic pressure increase resistance to movements providing strengthening to all muscle groups with basic and painless motions. The warmth of the pool along with the viscosity and turbulence of water interrupt the pain cycle allowing for exercise by even those with painful conditions that would not otherwise benefit from land based exercises. The bottom line is that an exercise program that allows for aerobic capacity increase, maximal ROM, and maximal muscle function without causing exacerbation of a condition absolutely has functional carryover to every day life.
Manual Therapy
And misaligned joints, all of which can cause pain, restricted mobility, and function. Having a therapist skilled with techniques to physically correct and aide in the healing of these problems, results in more successful and quicker recoveries. For many chronic pain patients these manual therapy skills may be their only hope for improvement.
TMJ Dysfunction
Is the least aggressive of manual therapy techniques traditionally used in physical therapy. It involves extremely light touch applied to different areas of the head and spinal tract including the “ tail bone” or the sacrum. Cranial sacral techniques are frequently time consuming so are some times limited in the clinical setting, but are very effective with many conditions including headaches, whiplash, and TMJ dysfunction.
