Northgate Health & Rehabilitation Center

My Mother was placed her by the Case Manager at the hospital while I was out of town and I was really scared because I was able to screen the facility first. She was there 3 days when I was finally able to go see her. I was surprised to see her in the rehab gym working with the therapist. When she was finished we were asked to attend a care conference. We were asked about what she likes to eat, how she like to spend her time, down to what time she likes to take her bath. These people are fantastic, they really care about her happiness. I couldn't have chosen a better place for my Mom. Monica the RN stays on top of everything.

Whoo-hoo for Northgate! A family member reported to ADON, see comes to see her father every day in the evening. She stated, "I am well pleased with the care and the staff on 400 hall. Brandon is a great nurse, Dymon and Sheena are some of the compassionate CNA"s I know!'

This Facility is a Hell.. Need i say more!! They do not supervise there patients/ residence. And staff or Nurses haveno clue how treat them. Or give them adaquit care what so ever. Or HYGIENE!! Family member fell out of his bed and was left for hours!! Facility is trying to cover it up. This Facility should be investigated toi full extent with so many horrible reviews.. I give no stars i give nothing to this establishment...but a death wish for your loved one!! The 1 star on professionalism goes to the therapy gentleman who gave my loved one a brief session of physical therapy and wished him well as the facility booted him after the fall..
They did not want to care for my family member and told me so.
Again do not go hear.
It was close by... But REGRET IT terribly!!!!!!

-WARNING - This Place "Northgate Rehab / 5757 N. Knoll" is one of the worst in San Antonio.., Full of Slackers from the Administration all the Way down to CNA's .., They put on a Show whenever Family or State Inspectors show Up.. They disappear in the Evenings and Weekends for extended hourly plus breaks,, and No One covers the Hall and patient Care.., There is No Sense of Teamwork because they are ALL SLACKERS.., If you ask Staff what is the appropriate amount of Time Given by Administration to Answer Resident Call Lights the Staff will shrink away from the Question Like Zombies Exposed to Sunlight.., - WARNING - "DO NOT TRUST YOUR LOVE ONES TO THIS MESS"

I had my dad placed in this facility July 16 2015 ,and seemed nice and Henrietta was most kind and helpful. I visited often and became aware that my dad wasn't being showered groomed dressed or getting much attention from staff.my dad would be asleep almost every time I would visit. My dad was on puree diet due to problem swallowing. My dad never received any therapy during 30 day stay and that was reason for going to this facility. This facility also upon sigNing him in let me know of trust fund that my dad would have access to pull out money for snacks purchase at outings and lubys when they would go out to eat .well they never opened account for him and sent 285.00 for payment for room and board along with his social security check. I had to get my dad out and now they have not wanted to return trust money and stated that his social security check he needs to live on is not being returned to him even though we already paid up for 30 days he didn't receive good care.

The staff are friendly, they seem caring.

my mother goes to visit my brother and is harrassed by the staff and they only let her visit 30 minutes everyday. she is 84 yrs old. he almost died a few weeks ago and his wife keeps sending him back to this death trap
