Causes for this condition are not always clear. Some trigger fingers are associated with medical conditions such as rheumatoid arthritis, gout, and diabetes. Local trauma to the palm/base of the finger may be a factor on occasion, but in most cases there is not a clear cause.
Primary Care
Attacks of gout and pseudogout can recur. When the episodes are infrequent, an NSAID or colchicine can be used as needed for flare-ups. If the epi-sodes occur more frequently, other types of medications are often indicated. The specific type of medication is best decided by your primary care physician and/or a rheumatologist. Patients with gout may need agents that decrease the production of uric acid, such as allopurinol.

Carpal Tunnel Syndrome
Carpal tunnel syndrome symptoms usually include pain, numbness, tingling, or a combination of the three. The numbness or tingling most often takes place in the thumb, index, middle, and ring fingers. The symptoms usually are felt during the night but also may be noticed during daily activities such as driving or reading a newspaper. Patients may sometimes notice a weaker grip, occasional clumsiness, and a tendency to drop things. In severe cases, sensation may be permanently lost and the muscles at the base of the thumb slowly shrink (thenar atrophy), causing difficulty with pinch.

In gout, crystals develop when patients over-produce or under-excrete uric acid. Certain medications can cause rapid changes in uric acid level. These include certain blood pressure medications, diuretics, intravenous blood thinners, and a medication used for transplant patients called cyclosporin. Alcohol also increases uric acid production. Hypothyroidism, heart disease, and kidney disease have also been shown to be associated with gout. Attacks of gout have been noted after injury, surgery, infection, and the use of contrast materials for x-rays.
Thyroid
We will assess the pattern and distribution of your symptoms, and examine for muscle weakness, irritability of the nerve to tapping and/or bending of the elbow, and changes in sensation. Other medical conditions may need to be evaluated such as thyroid disease or diabetes. A test called electromyography (EMG) and/or nerve conduction study (NCS) may be done to confirm the diagnosis of cubital tunnel syndrome and stage its severity. This test also checks for other possible nerve problems, such as a pinched nerve in the neck, which may cause similar symptoms.

Ultrasound
Therapy is often utilized to maximize motion. This might include exercises, scar massage, modalities such as ultrasound, heat, ice, etc., and splints that stretch the joint (static progressive or dynamic splints).
MRI
Magnetic resonance imaging (MRI) scan and electrodiagnostic studies are used to diagnose and locate the nerve entrapment.

Another distinctive golf injury is fracture of the hook of the hamate, one of the small bones of the wrist. The hook is the particular part of that bone that protrudes toward your palm, and is vulnerable to injury from the club on a hard hit to the ground as the handle crosses right over the bony hook when gripping the club. Hook of the hamate fractures may cause pain in the heel of the little finger side of the palm. If it irritates the adjacent ulnar nerve, it may cause numbness and tingling in the ring and little fingers. The tendons that bend the ring and little fingers are also adjacent to the hook, and movement of these fingers may be painful and give a sensation of “catching” or “clicking” if these tendons are rubbing on the fractured bone. Left untreated, the tendons can even gradually fray and rupture. In addition to physical examination, your doctor may obtain x-rays, but the fracture is often difficult to see on plain X-rays because of the overlap of the other small bones in the wrist. A special study called a CT scan is often used to visualize the area of the hamate hook in greater detail when a fracture is suspected. Treatment may consist of splinting or casting if the fracture is seen very early after injury. If seen late and there is continued pain, numbness and tingling, or tendon irritation, surgery is usually performed to remove the broken bone fragment.

Arthroscopy
The procedure is performed under general or regional anesthesia. A fiberoptic camera is inserted through a small incision, or ‘portal,’ in the elbow. The camera lens magnifies and projects the small structures in the elbow onto a television monitor, allowing the surgeon to accurately diagnose the condition. Several other small portals are used to allow the surgeon to place the camera in different positions to see different structures inside the joint and to place various small instruments into the joint to help treat various problems in the elbow. Sometimes elbow arthroscopy is combined with open procedures.

Pinched Nerve
Carpal tunnel syndrome (CTS) is a condition brought on by increased pressure on the median nerve at the wrist. In effect, it is a pinched nerve at the wrist. Symptoms may include numbness, tingling, and pain in the arm, hand, and fingers. There is a space in the wrist called the carpal tunnel where the median nerve and nine tendons pass from the forearm into the hand. Carpal tunnel syndrome happens when pressure builds up from swelling in this tunnel and puts pressure on the nerve. When the pressure from the swelling becomes great enough to disturb the way the nerve works, numbness, tingling, and pain may be felt in the hand and fingers.
Joint Replacement
Surgery is indicated when the patient either has too much pain or too little function. In most cases, the patient knows best and actually tells the doctor when it is time for surgery. The goal is to restore as much function as possible and to eliminate the pain or reduce it to a tolerable level. One type of surgery is debridement, in which the arthritic surface is removed and loose bodies are removed. Another approach is joint reconstruction, in which the degenerated joint surface is removed in order to eliminate the rough, irregular bone-to-bone contact that causes pain and restricts motion. Once the degenerated portion of the joint surface is removed, it may be replaced with rolled-up soft tissue, such as a tendon, or with a joint replacement implant. Which type of surgery is used depends on the particular joint(s) involved, your activities, and your own needs. We can help you decide which type of surgery is the most appropriate for you.

Arthroscopy
Arthroscopy is a minimally invasive procedure that allows a us to look inside a joint using small incisions (portals) and instruments the width of a pencil. Most people are aware of knee and shoulder arthroscopy, but the elbow joint has many conditions that can be treated with arthroscopy as well. The elbow is the joint that connects the upper arm bone and the forearm bones. Because the incisions used with elbow arthroscopy are smaller and disrupt less soft tissue than conventional open surgery, pain, swelling and stiffness are minimized and recovery is often faster.
Reconstructive Surgery
Recovery time varies considerably, depending on the severity of the injury, associated injuries, and other factors as noted previously. It is not unusual for maximal recovery from a wrist fracture to take several months. Some patients may have residual stiffness or aching. If the surface of the joint was badly injured, arthritis may develop. On occasion, additional treatment or reconstructive surgery may be needed.

Peroneal nerve arises from a branch of the sciatic nerve (present at thigh region) that surrounds the fibula. Fibula is the bone situated at the calf muscle which joins together with another bone called the tibia to form the knee joint. Peroneal nerve provides sensation and movement (stretching and lifting) to the leg, knee, feet, and toes. The most commonly observed condition of the peroneal nerve is 'Peroneal Nerve Entrapment' a condition caused due to compression of the peroneal nerve near the head of fibula, located below the knee. Conditions that compress the peroneal nerve include habitual leg crossing, braces around the nerve area, prolonged immobility which is observed in patients under anesthesia, fractures, knee surgery, tumors, trauma, and sciatic nerve lesions. The common symptoms are burning sensation in knee, numbness in the lower part of the body, pain, inflammation, loss of sensation in the peroneal nerve, paralyzed ankle, and weakness in lifting up the knee.

Arthritis literally means “inflamed joint.” Normally a joint consists of two smooth, cartilage-covered bone surfaces that fit together as a matched set and that move smoothly against one other. Arthritis results when these smooth surfaces become irregular and don’t fit together well anymore and essentially “wear out.” Arthritis can affect any joint in the body, but it is most noticeable when it affects the hands and fingers. Each hand has 19 bones, plus 8 small bones and the two forearm bones that form the wrist. Arthritis of the hand can be both painful and disabling. The most common forms of arthritis in the hand are osteoarthritis, post-traumatic arthritis (after an injury), and rheumatoid arthritis. Other causes of arthritis of the hand are infection, gout, and psoriasis. Osteoarthritis is a degenerative joint disease in which the cushioning cartilage that covers the bone surfaces at the joints begins to wear out. It may be caused by simple “wear and tear” on joints, or it may develop after an injury to a joint

Recovery from surgery includes physical therapy to regain motion of the arm. A strengthening program will be necessary in order to return to prior activities. Recovery can be expected to take 4–6 months.
Tennis Elbow
Lateral epicondylitis, commonly known as tennis elbow, is a painful condition involving the tendons that attach to the bone on the outside (lateral) part of the elbow. Tendons anchor the muscle to bone. The muscle involved in this condition, the extensor carpi radialis brevis, helps to extend and stabilize the wrist. With lateral epicondylitis, there is degeneration of the tendon’s attachment, weakening the anchor site and placing greater stress on the area. This can then lead to pain associated with activities in which this muscle is active, such as lifting, gripping, and/or grasping. Sports such as tennis are commonly associated with this, but the problem can occur with many different types of activities, athletic and otherwise.

Ankle Sprain
The treatment of a wrist sprain is guided by the severity of the injury. Similar to a sprained ankle, milder ligament sprains of the wrist may be treated with protected activity, supportive splinting or casting, strategies to minimize inflammation and discomfort, and gradual return to activity. Evaluation will help grade the severity of the injury, identify associated injuries, and determine the need for more specific diagnostic testing.
