Once the canal is thoroughly cleaned out, a rubber-like material, called gutta percha, is used to fill the space and then the tooth sealed with a composite material (filling). If the tooth is so badly damaged from decay that much of the structure has been destroyed, the dentist/endodontist may place small metal posts within the canal, around which s/he will build up the natural form of the tooth. A temporary crown will be placed. In cases like this, a permanent crown will be required, which can be placed at a second appointment. Until the permanent crown is placed you will usually be advised to stay away from chewing on that tooth to prevent that temporary crown from breaking and allowing the pulp to be re-infected.

Apicoectomy
An apicoectomy is considered when the risk of damaging the tooth further by approaching the infection from the chewing surface is considered high. In this case, the infection is approached from below the gum margin. While this can be done by your general dentist, these days the surgical procedure is either performed by an endodontist (two years further specialist training) or oral and maxillofacial surgeon (four years additional training).

Dentures
The only real alternative to root canal treatment is extraction of the tooth. This can be replaced with a denture, implant or bridge, none of which options is without need for follow-up care. In the long run, they will probably cost more than a root canal treatment.

Dental Implants
Obviously, if the infection is so widespread, or the tooth badly damaged, the apicoectomy will have to be abandoned and the tooth removed. Your dentist will try to avoid this circumstance, because repaired natural teeth are preferable to dentures, implants or bridges.

Dr. Jason M. Michel was born in Kansas City, Missouri, but much of his childhood was spent living overseas. Dr. Michel lived in Saudi Arabia, 9.5 yrs, and Guam, 2.5 yrs. He then returned to Kansas City to finish high school. After high school, he jumped right into a combined 6-year program at the University of Missouri-Kansas City School of Dentistry, earning a B.A. degree in Biology and my Doctor of Dental Science. He joined the US Army right after graduating dental school and served as a general dentist for the 2BDE, 4th Infantry Division for four years. After returning from a year deployment in Iraq (Nov 05-06), Dr. Michel moved to Fort Gordon, Georgia, where he obtained his certification in Endodontics (’09); then, he returned to Fort Hood, Texas, to serve for three years as a clinical endodontist and serve as the Chief of Endodontics for the US Army’s Advanced Education in General Dentistry program before leaving the military. Dr. Michel has been a private practicing endodontist in Texarkana since 2012. “My wife, Samantha, and daughter, Savannah, have been calling Texarkana home and are excited to explore a new chapter in our life as we move to the DFW area and become part of the Southlake Endodontics family.”
