However, in 15 to 30 percent of cases, AIED is “secondary” when it occurs as part of a larger autoimmune disorder that affects the whole body, such as rheumatoid arthritis, lupus, scleroderma, ulcerative colitis, or Sjogren’s syndrome. Audiologists are highly educated, health care professionals specializing in the diagnosis and treatment of hearing and balance disorders. Audiologists earn masters and doctoral degrees in audiology, pass rigorous national certification examinations, and complete a one year fellowship or residency in audiology prior to obtaining their state licensure. Their education includes a minimum of six years of college course work for the masters degree or eight years for a doctoral degree. To earn their degree, audiologists study anatomy and physiology, neurology, communication methods, speech and language development and disorders, acoustics, electronics, amplification, clinical methods, disorders of hearing and balance, counseling methods, and a variety of other topics related to hearing and balance. Audiologists are governed by their state licensure board, the American Speech-Language Hearing Association (ASHA), American Academy of Audiology (AAA), and Academy of Doctors of Audiology (ADA). These professional audiology organizations have strict ethical standards and require audiologists to complete extensive continuing education every year. (In contrast, a certified hearing aid dealer or hearing aid sales person only needs to have a high school diploma or GED and complete a brief, basic training course about hearing aids.) An audiologist's main priorities are ensuring the health of their patients relative to hearing and balance and helping patients and their significant others maximize the effectiveness of their communication.

If you have allergies, take your medications at the beginning of your flight. Over-the-counter nasal sprays or decongestants can also help air travelers to shrink the membranes and help the ears pop more easily. However, if you are pregnant, or have heart disease, high blood pressure, irregular heart rhythms, thyroid disease, or excessive nervousness, consult your physician before using these medications. Extended use of decongestant nasal sprays can also cause more congestion than relief, and even result in a type of addiction.

Based on the results of your hearing test and what your ENT specialist’s examination shows, as well as results from other potential tests such as imaging your ears with a CT or MRI, the specialist will make various recommendations for treatment options.

Computed Tomography
The specific type of surgery depends on what part of the ear is involved with the cholesteatoma. Sometimes the extent of the disease is clearly seen on the office exam. Other times imaging, often a CT scan, helps to define where the cholesteatoma is located. CT scans are a collection of X-rays that provide good detail on the bony anatomy of the ear. A hearing test, or audiogram, should be obtained. Other tests like an MRI or balance testing are less commonly required.

Irrigation or ear syringing is commonly used for cleaning and can be performed by a physician or at home using a commercially available irrigation kit. Common solutions used for syringing include water and saline, which should be warmed to body temperature to prevent dizziness. Ear syringing is most effective when water, saline, or wax dissolving drops are put in the ear canal 15 to 30 minutes before treatment. Caution is advised to avoid having your ears irrigated if you have diabetes, a hole in the eardrum (perforation), tube in the eardrum, skin problems such as eczema in the ear canal or a weakened immune system.

TMJ Dysfunction
In adults, common causes of earaches include otitis externa or swimmer’s ear and TMJ dysfunction. Swimmer’s ear is an infection of the ear canal and results from swimming in contaminated water or as a result of cotton swab use. In people with diabetes, otitis externa can spread far beyond the ear canal and can be life-threatening. Therefore, prompt treatment with antibiotic ear drops as well as cleaning of the ear canal with specialized tools available to an ENT specialist is critical. Another common cause of ear pain is due to referred pain from the jaw joint. This is usually due to the grinding of teeth during sleep. TMJ pain can be treated with ibuprofen, eating softer food, avoiding chewing gum, and using a nightguard.
