Male Infertility
Andrology is a branch of medicine that deals with the male reproductive system and urological problems. Andrology can cover a vast number of issues such as male infertility, sexual dysfunction, and male contraception.
Infectious Diseases
At the time of the initial visit, you'll see either one of our nurse practitioners or one of the physicians. During that time, we will review your medical records, so hopefully they will have been sent beforehand so we'd have a chance to review them prior to you getting here. We'll go through your medical record. We'll take a history that focuses on your reproductive system. We'll do a limited exam, depending on what the particular problem is, but we will do ultrasounds at most of our initial visits so that we can get an overview of the uterus and the ovaries to see if there are any glaring problems that we need to give attention to right away. Otherwise, all of our couples go through blood testing. We test both for infectious diseases. We will test the woman for a number of hormonal assessments that pertain very specific to the reproductive cycle. We do ovarian reserve testing, which means we're looking at the quality and quantity of eggs. We will do imaging to evaluate the uterus and the fallopian tubes to make sure that they are normal, or to determine if there are any abnormalities that need to be addressed. If a male partner is involved, we will do semen analysis. After this battery of tests is complete, then we will have a couple return for a plan of care visit, at which time we go over the results of all the tests, and we discuss what our recommendations for treatment will be. We will question them about what their habits are. We concern ourselves not just with the medications that they're taking, but are they smoking? Are they drinking too much? Is there any recreational drug use? I think it goes without saying that the patient needs to bring their healthiest self to us if they can. And to the extent that they aren't, we help them to identify the areas that they need to pay attention to, like diet, exercise, clean eating, getting enough sleep. What they have control over is taking as good of care of themselves as possible. A patient knows if they're smoking too much, or they're drinking too much, or if they're smoking marijuana, or doing other recreational drugs. They know that that isn't going to benefit them. Most of the patients who come to us know that. They're very motivated to do all that they can to help this process to work. One of the things that I try to emphasize to patients when they come here is that this is a shared problem. It's not unusual for a woman to think that this is her problem, that she is the one who's damaged, she is the one that needs to be fixed. We explain to them that infertility is a couple problem. It is a couple problem. It is not one individual's problem. I'd say about 40% of the time the problem is attributed to the woman. About 40% of the time it's attributed to the man. About 20% it's a combination of the two. The fact of the matter is that they're working as a unit. We can't have one without the other. So we try to emphasize that and to make sure that both are appreciating their role in this, whether

Dr. Yao received her Bachelor’s degrees in both biology and nursing from Creighton University, followed by a Master’s Degree as a Women’s Health Nurse Practitioner from the College of Nursing at the University of Nebraska Medical Center. She then joined the faculty at the College of Nursing at the University of Nebraska Medical Center, where she served as the Continuing Nursing Education nurse planner and clinical instructor in obstetrics and gynecology. During Dr. Yao’s appointment as the nurse planner, she collaborated with numerous medical and professional disciplines to develop and implement continuing education programs for healthcare providers. In 2017, Dr. Yao received her Doctor of Nursing Practice from the University of Nebraska Medical Center.

Obstetrics
While in medical school, Dr. Constance volunteered as the Executive Director of a free medical clinic providing general medicine, obstetrics, and gynecological care to uninsured patients in Kansas City, Kansas. She also sat on the board of multiple organizations focused on improving access to healthcare across the Kansas City Metro area. During residency, Dr. Constance was recognized for her devotion to teaching with the Medical Student Voice Award. She also received the Making a Difference Clinical Award for outstanding patient care during her fellowship training at Michigan Medicine. Dr. Constance has conducted research on fertility preservation, recurrent pregnancy loss, and transgender hormone therapy. Reciprocal IVF allows both partners to have a biological connection to their child: one partner genetically, and the other through pregnancy and childbirth. One partner will go through the process of stimulation and egg retrieval. After fertilization with donor sperm, the embryo is transferred to the uterus of the other partner.

She attended the Arizona College of Osteopathic Medicine, and completed her general Obstetrics and Gynecology residency as well as her fellowship in Reproductive Endocrinology and Infertility at the University of Oklahoma Health Sciences Center. During fellowship training, she also obtained a Master's Degree in Clinical and Translational Science through the University of Oklahoma College of Public Health. Dr. Weedin's research interests include how lifestyle and mental health affect infertility.

Victoria Maclin: I consider us very lucky because we have all of the benefits of being in a private practice, but we also have the opportunity to teach our residents. The university sends the residents to our facility, and they do their reproductive endocrinology education with us. And so it gives us an opportunity to have a foot in both worlds because while I love private practice, I love taking care of patients. I also love being able to give back by teaching. What we teach goes on, and then they teach the next person, et cetera. So it has a ripple effect, and it's nice to be able to participate in that. Ultrasound
During a patient's first visit to Heartland Center for Reproductive Medicine, our team will evaluate your medical record, perform a limited exam using an ultrasound, and gain an overview of your reproductive health. Patients will also undergo blood testing for disease and any other conditions that could affect fertility, as well as egg testing. Chemotherapy
Certain cancer treatments, such as chemotherapy and radiation, can be harmful to both male and female fertility. By preserving reproductive tissue, patients are increasing the likelihood that they will be able to conceive following their recovery. Fertility preservation was originally intended for cancer patients, but in recent years the definition has been expanded to include patients with certain genetic disorders, those who require repeated surgeries on their reproductive organs, and women who wish to preserve their eggs before their fertility window closes.

Minimally Invasive Surgery
In vitro fertilization, or IVF, is one of the most common methods of assisted reproduction and one of the most successful. After taking medications to stimulate the ovaries, the prospective mother will undergo a minimally invasive surgery to retrieve the eggs from the ovaries. Once fertilized in the lab, the fertilized eggs (embryos) are placed in the uterus.
Robotic Surgery
We're particularly proud of the fact that we're able to provide state of the art technology. One of the developments that's made a huge difference in what we can do with reproductive surgery is robotic surgery. Using a robot you're able to do in a minimally invasive manner procedures that would typically require a larger open incision. With the help of the robot you can do precise movements with the ability to have better magnification and better manipulation of the instruments almost as if you were holding the instrument in your hand. So it's possible to do complex work within the abdomen that ordinarily you would need to do through a larger incision. As a result, because we can do this through smaller incisions the patient is able to spend less time in the hospital. She's able to recover much more quickly and get back to her normal activities sooner.
Laparoscopy
Reproductive surgery is a particularly important part of our practice. Sometimes there are anatomical problems that will prevent pregnancy, for instance, scar tissue within the uterine cavity or growths within the uterine cavity or within the uterine muscle that will prevent the uterus from functioning as well as it can. So often we will need to do a procedure to repair or enhance the uterus so it's able to function more appropriately. There are other surgical conditions such as endometriosis, a problem that can cause extensive scarring within the pelvis and prevent the fallopian tubes and ovaries from communicating well with each other. There are also other problems that endometriosis can cause which will impair fertility, such as inflammatory changes and such. One of the things that we're particularly proud of is our ability to do minimally invasive surgery. Most of the surgery we do can be done through small incisions, which allow faster recovery time. We also do robotic surgery, particularly for treatment of uterine fibroids, which allows us to do a procedure that ordinarily would need to be done through a larger incision, requiring longer hospitalization, longer recovery time. Because we can do this with the assistance of a robot through smaller incisions, the procedure can be completed in a more efficient manner. These developments have really helped to enhance the service that we can provide. One of the problems that we frequently are confronted with is congenital abnormalities of the uterus where the uterus has not developed as normally as it should. And there are procedures that we can do to repair it and enhance it so that it'll function better for achieving a pregnancy and allowing a pregnancy to progress to its full term. One example is a uterine septum. A uterine septum is a wall of tissue within the uterine cavity that can be easily removed through a minimally invasive procedure called hysteroscopy, and it can make the difference between a miscarriage rate as high as 80% to a live birth rate as high as 80%. There are other conditions that will also require surgery, such as damaged fallopian tubes. If IVF is a required treatment for a patient who has tubal blockage, it's usually necessary that we remove the fallopian tubes before in vitro fertilization can be done. That can be done through a minimally invasive procedure called laparoscopy, which allows the patient to recover relatively quickly and be back to work within just a few days. Minimally invasive surgery allows us to go in and correct the problem and make it so that the patient can go on with the process of trying to become pregnant.
