Meet Dr. William Nolan, an OB-GYN with UPMC Divine Mercy Women’s Health in Carlisle, Pennsylvania. This group was formerly with Holy Spirit Hospital, and it was a Catholic medical practice. It has since merged with the University of Pittsburgh Medical Center of Central PA, but unlike with other secular institutions, they have been allowed to continue to practice in a way that is in accord with their ethics.

In the late 1980s, Dr. Veasy Buttram, a well-known reconstructive pelvic surgeon initiated a one-year fellowship program in reproductive surgery. The program was developed because he recognized that most graduating residents in obstetrics and gynecology were not fully trained in modern reproductive (infertility) surgery. This was the first of a group of ad hoc fellowship programs which were developed mostly in non-university hospitals by private physicians. This has now evolved into an advanced endoscopy fellowship training program sponsored by the Society of Reproductive Surgeons and the American Association of Gynecologic Laparoscopists.

The reproductive endocrinology programs do not represent universal ethical principles which are applicable to all patients. The embryo-destructive nature of IVF procedures excludes a large population of patients from being assisted by these approaches. Furthermore, the artificial reproductive technologies are oriented towards “skipping over” the underlying disease process and attempting to treat the reproductive abnormality by replacing it with an artificial approach. All in all, the overall success rates with such approaches have remained relatively stagnant over the last 25 years.

Having said all this, however, we are in a unique position to reach young people of faith who do not go floating in the direction of the various breezes. When you think about it, that is really incredible, and, in many ways, exciting. Our fellowship is one year in duration. Physicians learn how to perform surgical procedures, which can be quite extensive. Because of research completed here at the Institute to help prevent the formation of scar tissue and adhesions from such a surgery, we developed a new surgical procedure called pelvioplasty. This is a plastic surgery approach to the pelvis, really significantly reducing the adhesion formation and allowing us to complete the types of extensive surgeries often necessary. In fact, the Institute and its reputation have grown so much in the last 15 years; the level of cases we see — the severity of them — has also grown. This is mostly because patients have seen other doctors in their local communities who have performed surgical procedures but do not know how to perform these procedures or are unwilling to take the time to do so in such a way as to eliminate scar tissue formation.
