Plantar Fasciitis
The plantar fascia is a band of connective tissue on the plantar surface of the heel that plays a large role in maintaining the normal architecture of one’s foot. Plantar fasciitis is a common clinical condition caused by overuse or injury of the plantar fascia and is defined as traction degeneration of the plantar fascial band at its origin on the medial tubercle of the calcaneus. Inflammation, fibrosis, and decreased vascularization of the fascia occur, causing symptoms of heel pain. Other symptoms that may occur include burning in the sole of the foot, recurring foot pain that is especially aching in the morning or after sitting, or heel pain after beginning a new exercise routine.

You may be the first person to get a consultation request, either through a page from the floor, the emergency room, a message on Athena, by a hospitalist, vascular surgeon, or another doctor, or by monitoring the hospital in-patient list. See the consult in a timely manner, add the patient to the hospital list, and notify the appropriate attending. Begin the order set appropriate for the patient, including vascular, wound care, and weightbearing status orders. If the patient is a surgical emergency, manage appropriately, add to the schedule, and communicate with the attending surgeon so that there are no delays.

On my first visit I was convinced that Dr. Lam was the man for the job. Dr. Lams open and warm personality, his explanation of the problem, his proposed solution and his reputation as a surgeon was very reassuring. It was determined that the right toe was a perfect candidate for this break through joint replacement procedure. The left toe at stage 1 is not quite ready. Dr. Lam explained how he would remove the calcium buildup from the medial (foot) part of the joint and replace the joint in the toe with the implant.

Fellow will receive an extensive amount of exposure to MIS surgery (HAV, Tailors, Weil, Hammertoes, Haglunds,Calcaneal Osteotomies, etc), arthroscopy and arthroscopic fusions (STJ/AJ), Nerve transposition, Dellon release, rearfoot/ankle fusions and reconstructive techniques, including tendon transfer; use of internal and external fixation; management of complicated patients with comorbidities.

For Achilles tendonosis / tendonitis we have been able to avoid surgery on a majority of our patients even those with large heel spurs! The spurs remains but the pain is gone, thus, no further intervention is required. We have had patients that were scheduled for surgery at another facility but after Extra Corporal ShockWave Therapy the pain fully resolved.
