Lifestar Case Management Services, Inc. has been active since the beginning of 2012. We are an agency approved and audited annually by Florida Medicaid. We offer "Adult Mental Health, Targeted Case Management (TCM) Services" Lifestar CMS, Inc. has its main office based in Miami Dade County, privately owned and operated. The main office is established in quality office space on Hialeah, The city is a strategically centric location to provide services to different cities within the county. To receive Targeted Case Management Services, the Client must be Medicaid eligible on the date of service * Client must be 18 years old or older. * Have a mental health disability (i.e. severe and persistent mental illness) that requires advocacy for and coordination of services to maintain or improve level of functioning that based upon professional judgment. * Requires services to assist in attaining self-sufficiency and satisfaction in the living, learning, work and social environments of choice. * Lacks a natural support system for accessing needed medical, social, economical, legal, educational and other services. * Requires ongoing assistance to access or maintain needed care consistently within the service delivery system. * Is not receiving duplicate case management services from another provider. Consumers of our services are those individuals and families in need of Case Management and Social Services. These clients are usually referred by other health care professionals such as physicians, neurologists, psychiatrists, insurance companies and health care facilities. Even though our agency was recently opened, it has already developed an excellent reputation with many of these professionals, through the work of our administrative staff and our case managers, who have been providing Case Management services keeping in mind the clients welfare as their main priority. Lifestar CMS, Inc. Primary founder is Jenny B. Ibarra, a Foreign Medicine Doctor, who has been in the field of Case Management for several years and our Director is Wilson E. Solorzano, BA / MSW/ MBA / Registered Clinical Social Work Intern, who has many years in the field of Case Management and Social Work as well. The rest of the administrative team is made up of professionals in the area of Psychology, Clinical Social Work, and Business Administration. Our Case Management team is made up of Foreign Medicine Doctors and Sociologists who are aware of the needs elderly population have and are well trained to address these needs in a professional and courteous manner.
