Countryside Home is a nonprofit, more than 100 beds skilled nursing care facility that is a part of Jefferson County in Wisconsin. The facility s campus features an atrium, work activity center and computer rooms. It includes a rehabilitation gymnasium that offers inpatient and outpatient services. Countryside Home also provides mental health, pharmacy and social services. The facility s each household includes a kitchen, dining room and outdoor courtyard.
