Founded in 1988, Community Care serves individuals with disabilities throughout Chickasaw, Clinton, Jones, Jackson, Scott and Marion counties in Iowa. The agency offers home health care, community employment supported community living and mental health services. Community Care also offers home- and community-based services. It provides online links to educational resources, such as the Iowa Department of Human Services, Center for Medicare and Medicaid Services and National Alliance on Mental Illness.
