If you have already been evaluated by another physician and have records related to the condition you are seeking treatment for, please have them sent to our office by completing the release of medical records form by selecting the link below. Send this form to your provider so they can fax the records to our office prior to your visit. We try to accomplish as much as possible during your initial visit. Please do not take any antihistamine containing medications for at least three days before your appointment. Antihistamines will interfere with allergy testing if that is required. Your first visit may require as much as two hours if testing is involved.
