When patients enter the dentist office with a "cracked tooth" it is generally because part of the tooth has actually fractured off leaving the dentin or even nerve exposed.Â In the vast majority of cases, the tooth cracks because it already had a large dental filling in it, and the remaining real tooth structure was not strong enough to handle the forces of chewing.Â Without a doubt, this happens becauseÂ a filled tooth is substantially weaker than an unrestored or "virgin" tooth.Â When doing a filling, it is the dentists job to remove all of the decayed tooth structure and replace it with restorative material.Â In the past, this was usually done with amalgam.Â Currently, dentists prefer composite resins which are tooth colored and actually bond to the tooth.Â This helps hold everything together, but most of the stress the tooth receives in function must be managed by the tooth structure left over and not the filling.Â When a tooth has a large dental filling and even more of it fractures away, there is even less healthy tooth structure to handle the forces of mastication.Â Since there was not enough tooth to handle the forces with the existing dental filling in place, it is very unlikely that a larger filling (replacing the old dental filling and the newly fractured piece) could be placed that would last very long.Â In these cases the dentist is forced to place a dental crown on the tooth.Â This is a cast or milled restoration that fits over the entire tooth and holds it all together.Â They are extremely strong and designed to have the same shape and appearance of the original tooth.Â They are cemented over the remaining healthy tooth structure and should be extremely precise so that no bacteria can get back under the dental crown.
Veneers
Teeth whitening techniques can be divided into two basic categories, at home and in office whitening. Otherwise known as professional teeth whitening. Â We will go into the pros and cons of each, but it is important to realize that this article only refers to the techniques for whitening tooth structure through chemical means to get a more esthetic color for the actual tooth enamel.Â This does not includeÂ "veneers" or "composites" which are restorations which can change the color and shapes of teethÂ by adding material to them.Â In fact, it is Â not possible to change the color of restorative materialÂ so if a patient has restored front teeth they will not change color with any tooth whitening technique professional or otherwise.
Crowns
The amount of tooth that has been destroyed determines what options the dentist has to restore the tooth. As a general rule, the less tooth that is decayed the less expensive treatment will be. For this reason early detection is the best case scenario. There are basically two ways for your dentist to restore decayed tooth structure. The first is a filling and the second is a crown. â€œFillingsâ€ are basically placed in a tooth after the decay has been removed. There must be enough healthy tooth structure left after decay removal to hold the filling material and also bear the stress of grinding your teeth while chewing. If too much tooth has decayed the only option is to place a crown over the entire tooth. This is a much stronger restoration that holds the tooth together and bears the stress of chewing much better. Unfortunately, it is also much more expensive than a filling.

Restorative Dentistry
Restorative dentistry is the process of removing unhealthy tooth structure or teeth and restoring the form and function of the mouth. The most common destructive forces that adversely effect teeth include decay, trauma, and periodontal disease. Tooth decay is referred to as dental caries. It is the biological process of bacteria sitting on a tooth and eating what bacteria eat, and excreting acid by- products which over time eat away at tooth structure. The sole purpose of brushing and dental flossing your teeth is to remove all the bacteria from your teeth so that it can not produce acid to eat away at your tooth structure. The normal progression ofÂ tooth decay is a slow process because the protective enamel covering your teeth is usually hard enough to slow the effects of bacteria produced acid. Problems arise when the bacteria sits in the same place for prolonged periods of time and eventually dissolve the hard enamel and bacteria gets into the softer dentin inside your tooth. Once this happens the progression of â€œcavitiesâ€ dramatically increases.

Root Canal Treatment
The last five years have brought interesting changes to the success of root canals. The evolutions of microscopic cameras and scopes used in arthroscopic surgery have been adapted for use in root canal therapy. The extremely small camera lenses can be inserted into teeth and magnify the images so that additional nerve canals can be found, and cracks in teeth can be diagnosed. Before this technology, the anatomy of teeth was assumed to be fairly uniform for each type of tooth i.e. lower molar, anterior incisor, maxillary bicuspid etc. Technology has proved that our tooth anatomy is actually quite varied and microscopes allow the visualization of additional nerve canals to be spotted and treated. The microscopes also allows for cracks to be spotted within the root structure. This is very important because any tooth that has vertical fractures in the root can not be saved whether they are root canalled or not. This technological advance prevents patients from getting root canals that are destined to fail anyway.

Dentures
There is no doubt in my mind, that if a patient had lost 28 of 32 teeth already, there was little chance his partial dentures would last very long especially since they were holding on (and stressing) his remaining teeth. Strangely, he didnâ€™t get any better at taking care of his teeth either. Frankly, I am surprised his $12,000 investment lasted 4 years. The original dentist should have made the options available to this patient but realized his plan was doomed from the start. Brand new dentures would have saved this patient $12,000 and the results would have been better and more predictable. Always find a dentist who welcomes open lines of communication and is informative about your options. Look for someone who is generally concerned about your welfare and understands how much treatment costs. Dentistry is expensive, but look for a dentist who wants to deliver real value for your money. It is always someone who wants you to be a patient forever.

Gum Surgery
We now can utilize mini implants which usually do not require tissue surgery or bone grafts and can be placed in about 30 minutes. They are used to replace lost teeth or stabilize dentures just like conventional implants but require no gum surgery or excessive bone removal like previous implant systems. The additional benefits are healing time, less complications, and a cost that is about half that of conventional implants.Â
Tooth Extractions
There are three classifications of wisdom teeth.Â The first is erupted wisdom teeth.Â This just means that it has broken through the gingiva.Â It may just be partially exposedÂ or even completely out of place.Â The second classification of wisdom tooth eruption isÂ soft tissue impaction.Â This just means that the tooth is still covered by the gums so itÂ is not visible inside the mouth.Â The third classification is boney impaction.Â This refers to a tooth that is still covered by bone.Â Wisdom teeth can be extracted no matter what their classification.Â One of the benefits of panalypse x-ray is that your dentist can evaluate the wisdom teeth and their roots to see if the extraction will be an easy one.Â If it is tricky,Â or ifÂ multipleÂ wisdom teeth are being extracted we usually send patients to an oral surgeon.Â Oral surgeons utilizeÂ mild general anesthesia to make the experience easier on the patient.Â Every case is unique unto itself.Â Most patients have heard horror stories of chipmunk cheeks, but in most cases there is little swelling or discomfort.Â Naturally the boney impacted teethÂ take more healing time than theÂ simple removal of an erupted tooth.Â Issues which complicate removal of erupted wisdom teeth include multiple roots which diverge in different directions, proximity to nerves in the jaw, or impaction's which place the tooth very close to the sinuses.Â Â Generally speaking, wisdom tooth extraction is much easier on the patient than is commonly thought by the general public.

Teeth Whitening
All "teeth whitening" is currently done by theÂ chemical carbamide peroxide.Â It is the active ingredient in all tooth whitening systems including over the counter, at home, and in office techniques, professional teeth whitening.Â Â The difference between each system which dramatically effects its success is the means of applyingÂ carbamide peroxide to the teeth and the concentration of the chemical used.Â If you think about it, for a chemical toÂ lighten the very hard enamel of a tooth it has to be fairly caustic.Â It needs to permeate the enamel to lighten it.Â It also follows, that the more concentrated the tooth whitening gel, the more harmful it could be to the soft tissues of the mouth like the gums and lips.Â As aÂ rule, over the counterÂ tooth whitening like "strips" orÂ "whitening toothpaste" have very low concentrations of carbamide peroxide.Â I have seen products that range from 1-8% carbamide peroxide in over the counter systems.Â This is very low and it makes senseÂ because users are just placing it themselves without protecting the gums and lips.Â If it was highly concentrated it would burn the soft tissues causing pain, swelling, andÂ sores.Â From my experience, these systems are ineffective and appear to be low costÂ Â but require repeated use to get only minor results.
Cosmetic Dentistry
I believe that all modern dentistry is "cosmetic dentistry."Â In my twenty five years of practicing dentistry I have never had a case where I didn't care what the patients results looked like.Â Every dental patient deserves a presentable smile and is a walking billboard for your services.Â In some cases financial concerns, health concerns, or just rampant decay hamper your finished results.Â Even in these rare compromised situations,Â all dentists want the best esthetic results possible with every restoration for the patient.Â Some dentists may think of cosmetic dentistry as something along the lines of plastic surgery.Â They diagnose a complete overhaul of all the exposed tooth structure to give it a new shape, texture, and color.Â These are exciting and profitable cases and I enjoy doing them because dental patients are so delighted by theÂ results.Â The fact is that few patients have the financial ability or the desire to undertake such a makeover.
