Male Infertility
Male factors are found to be the cause of infertility about 30% of the time. In another 30-40% of couples both female and male factors contribute to the coupleâ€™s infertility. These are surprising facts for many couples to learn â€“ especially those who mistakenly believe infertility to be a womanâ€™s issue. Therefore, it is essential that men be tested. Male infertility is diagnosed by a semen analysis.

Infectious Diseases
All sources of donor sperm (whether obtained via a commercial bank or friend/relative) undergo mandated FDA screening for genetic and infectious diseases to ensure that there is no risk of passing down an identifiable illness. If you are using a fresh sample from a known donor, they will have to undergo this testing within 7 days of insemination of IVF retrieval. If you are using sperm from a bank, then the donor will have already been tested for genetic and infectious diseases.

Ultrasound
The third common feature of PCOS is what is called polycystic ovaries. This can be seen on a transvaginal ultrasound. This is actually a misnomer as the ovaries of women are not really full of cysts but rather ovarian follicles that each contains an egg. All women have these follicles in their ovaries and each month a group of follicles start to develop, with one going on to be the dominant follicle that ovulates the egg. The ovaries of women with PCOS may contain many small follicles that do not go on to ovulate an egg each month. These follicles fail to develop normally because of the hormonal imbalances in PCOS. Because the ovaries do not grow and ovulate an egg each month, women with PCOS may also experience difficulty getting pregnant.

Hyperprolactinemia is an excess of brain hormone called “prolactin.” This is an essential hormone that helps to regulate milk production for women who are breastfeeding. However, the body can occasionally overproduce prolactin; this excess can lead to irregular ovulation and infrequent menstruation in women. As it is often an asymptomatic condition, we screen all women for hyperprolactinemia. If your levels are found to be high, we will often repeat the test when you are fasting (have not eaten anything). If the level is still high, we may recommend an MRI or CT of the brain to determine if there is a benign growth in the brain that is causing the excess prolactin. Hyperprolactinemia is generally treated with an oral medicine that helps to normalize the prolactin levels and decrease the risk for irregular ovulation/menstruation.

Gestational surrogacy was developed initially as treatment for women who were either born with an abnormal uterus, or no uterus at all, or who had undergone hysterectomy. Most of these women are otherwise fertile, meaning that their ovaries and egg reserve are completely normal; they simply lack a proper organ in which to gestate their embryo. Today, the indications for surrogacy have expanded to include women in whom pregnancy would be dangerous due to medical conditions and women who suffer from recurrent pregnancy loss due either to immunological disorders or for other reasons and who do not respond to more traditional interventions. Some women also use donor eggs to establish pregnancy in the GC, but in gestational surrogacy the egg donor and the GC are never the same person (due to the legal and ethical quandaries that might ensue from such an arrangement). This is also the case for same-sex male couples who wish to start a family and who need both a donor to provide the egg as well as a GC to carry their child. Clearly there are some single women and men who have also had children in this way.
Laparoscopy
The diagnosis of anatomical infertility is not always straightforward. Certain aspects of a woman’s history may suggest an anatomical blockage, but seldom is this revealed on physical exam. Most often, the diagnosis is determined on the basis of x-ray (hysterosalpingogram), sonohysterogram, diagnostic hysteroscopy and/or a laparoscopic evaluation. If confirmed, therapeutic modalities may include a variety of surgical procedures, including operative laparoscopy, operative hysteroscopy or microsurgical reconstruction via laparotomy.

Laparoscopic Surgery
Laparoscopic surgery is done through a thin, illuminated telescope that is placed through the abdominal wall. It may be simply diagnostic, as when it is used to confirm the normality of the pelvic structures, or therapeutic, as when it is used to correct internal pelvic problems. Laparoscopy can also be used to treat endometriosis and pelvic adhesive disease that may contribute to pelvic pain. Endoscopic video cameras and lasers enable experienced surgeons to perform even complicated reconstructive procedures on an outpatient basis. In many circumstances, laparoscopy may be done at the GENESIS on-site surgical suite.

Lesions
Endometriosis is a common condition that can make it more difficult to achieve pregnancy. It is thought that endometriosis most likely results from shedding of the uterine lining (the endometrium) into the pelvis, rather than shed through the cervix and vagina as usually occurs with menstrual periods. This displaced endometrium can attach to tissue surfaces within the pelvis and begin to grow as nodules or lesions, resulting in pain and over time, scar tissue/adhesions. In patients with more advanced endometriosis, this scar tissue can disrupt the ability of the egg to be released from the ovary and enter the fallopian tube, leading to infertility. Some patients with endometriosis may have symptoms (such as painful menses or discomfort during intercourse), while others may be asymptomatic. In fact, some patients who seek care for fertility issues are found to have endometriosis during their evaluation, even though they had no signs or symptoms. As fertility specialists, we help to treat the symptoms of endometriosis and to help patients achieve pregnancy, if desired.
