Teeth Whitening
We are a provider of services for most PPO plans that employers offer to their employees. But there are a lot of people out there that do not have the opportunity to participate in these plans. Therefore, Dr. Colvin wanted to offer something comparable to her patients. So we formulated a plan very similar to those we already provide in office. These are individual based plans and are valid for a period of 12 consecutive months. This plan does not roll over and patient will have to renew after a period of 12 consecutive months. If for whatever reason, patient does not have (2) cleanings within their 12 consecutive months, they forfeit that procedure with no refund of money and it does not roll over past 12 months form the original date of purchase.
