Glaucoma
Of new, fragile blood vessels on the surface of the optic nerve or retina. These abnormal blood vessels are called neovascularization and have a tendency to bleed into the vitreous cavity causing a vitreous hemorrhage. The neovascularization may also cause scar tissue to form on the retina which can pull on the retina leading to traction retinal detachment. Sometimes, neovascularization also grows in the front of the eye on the iris which can lead to a very severe form of glaucoma.
Ophthalmology
David E. Barañano, M.D. Ph.D. received his B.S. from Yale University in 1996. He earned both his M.D. and a Ph.D. in neuroscience at the Johns Hopkins University School of Medicine in 2004. He completed his medicine internship at the University of Maryland – Mercy Medical Center and his ophthalmology residency at the Wilmer Eye Institute at Johns Hopkins. He completed a two-year fellowship in vitreoretinal disease and surgery at Emory University. After completion of his training, Dr. Barañano returned to Baltimore to join the faculty at the Wilmer Eye Institute. He was chosen to serve as the 102nd Assistant Chief of Service (Chief Resident), supervising and training the 21 physicians of the Wilmer residency. Afterwards, he joined the Retina Division of Wilmer, where he taught retina fellows and residents and led Wilmer’s pediatric retina care. He was delighted to join Dr. Thompson and Dr. Sjaarda at Retina Specialists in 2013.
Macular Degeneration
Since our beginning, we have been on the cutting edge of advances in treating some very common eye conditions. We have been involved in clinical trials that have led to approval of three major treatments for wet age-related macular degeneration, and we are now enrolling or will soon begin to enroll patients in three more studies.
Cataracts
One possible complication is dislocation of cataract fragments into the vitreous cavity. The common symptoms from dislocated lens fragments include large floaters, blurred vision from fluid leakage in the macula or abnormal inflammation that may cause elevated intraocular pressures. This problem can be treated by vitrectomy where the cataract fragments are removed, restoring vision.
Cataract Surgery
Some eyes may develop problems following cataract surgery that may benefit from additional surgery by a retina specialist.

Diabetic Retinopathy
Diabetic retinopathy includes a number of changes in the retina which can develop as a result of diabetes (either type 1 or type 2). The earliest signs are small red dots in the retina called microaneurysms which represent small pouches on the smallest retinal blood vessels called capillaries. These microaneurysms can leak fluid from the blood stream (serum) into the retina which can lead to swelling in the retina (macular edema). Later, retinal hemorrhages and white spots called cotton-wool spots may develop in the retina. With more advanced retinopathy, small irregular capillaries may develop called intraretinal microvascular abnormalities (IRMA) and venous beading where the retinal veins will have areas of dilation next to areas of normal blood vessel caliber. All of these changes are called non-proliferative diabetic retinopathy. Some eyes advance to proliferative diabetic retinopathy which is characterized by the development
