Some women never develop breasts to the degree that makes them feel well proportioned. They often have difficulty fitting into clothes, especially summer clothes and bathing suits. They may seek surgery for the simple reason of filling out their clothes correctly. Some wish to feel more balanced, to match the rest of their figure. Other women come in because they once had breasts that they were happy with but aging, pregnancy, breastfeeding, and weight loss have taken their toll. These women often just want to regain what they lost through these circumstances.
Primary Care
Interfering with mammograms - implants do block x-rays and produce a shadow on mammograms. To get around this, mammogram techniques are modified for women with implants. A displacement technique (Eklund technique) is used to push the implants out of the way, and additional views are taken. Most, but not all, of the breast tissue can be seen. It is theoretically possible that a small lump might be missed, and if this lump were cancerous, this could result in a delay in diagnosis and treatment. Studies have compared breast cancer patients who had breast implant surgery with those who never had implants. No statistical difference has been found in the results of the cancer treatment between the two groups. Capsular contracture, by making the breasts difficult or impossible to compress, can compromise mammography. Breasts that stay soft can be examined by mammography more effectively. This is why I feel compliance with the program of breast massage, taking vitamin E, and using a compression garment to help keep your breasts soft is so important early on after surgery. Follow your primary care provider’s recommendations for obtaining screening mammograms.

Suzanne joined us in 1996 shortly after the move to our new facility. She came to us with prior experience in urology, ophthalmology, and home health. In 1995 She obtained her RN degree at Lake Sumter Community College. She joined us as a clinical/surgical nurse, but years ago she assumed the position of director of our ambulatory surgery center. She oversees all aspects of running our surgical suite; from maintaining our licenses and certification, to insuring that equipment is maintained and all preparations for surgery are in place. She circulates or assists during surgery. In 2008, Suzanne obtained her Risk Management license from the University of Florida and is our Risk Manager. In 1998, She traveled to Brazil on her first medical mission and assisted Dr. Bosshardt during a week of intensive reconstructive surgery. The experience was capped off with a trip to the Amazon. In 2007, she traveled to Venezuela with Dr. Bosshardt in a second medical missions trip doing reconstructive surgery. A central Florida native, Suzanne is married to John, an attorney and has two children. She enjoys boating, cooking, and hiking.

Endoscopy, surgery via small incisions using a lighted device called an endoscope, is the proverbial hammer in search of a nail. As with lasers, it has been over-hyped and some practitioners have tried to promote it for procedures where it provides no real advantage and actually creates problems. While it has revolutionized many areas of surgery, it has had limited application for plastic surgery. Much of what plastic surgeons do involves the need for larger incisions and this effectively negates one of the biggest advantages of endoscopic surgery.

You may be referred to plastic surgery for a broad range of hand problems. We do lots of trauma care involving hand injuries. These include such things as tendon repair, repair of hand fractures, and repair of complex soft tissue injuries of the hands and fingers. In the area of elective surgery, you may be sent to a plastic surgeon for removal of tumors or skin cancers of the hands; for problems related to Dupuytren's contracture, an unusual condition in which fibrous bands in the palms gradually cause the fingers to bend and remain bent; for carpal tunnel surgery to relieve pressure on nerves in the wrist than can affect hand function and sensation, for trigger fingers; and for ganglions of the fingers and wrist.

You have the choice of silicone gel or saline implants for their surgery. I still feel that for most women, saline implants (filled with sterile, salt water) provide a result that is on par with silicone gel implants and these are my “go to” implants for most patients unless I feel that the gel implants are better for a specific individual. Saline implants are excellent implants and provide wonderful results for most patients. One advantage to saline is that if the implants ever rupture or leak, the saline is harmlessly absorbed by your tissues and eliminated. Replacing a leaking implant is quite simple and easy to do. Women with unusually thin skin and minimal breast tissue, may benefit from the gel implants because they tend to produce less visible rippling. This fact may be more important when the implants are under thinner tissue. The new gel implants have a more viscous gel that will not run, so leaks should not be as messy as in the past. One of the potential problems with gel implants is difficulty detecting leaks and it is recommended by the FDA and implant manufacturers that women get an MRI three years after implantation and every two years after that to check for leaks. This is a suggestion not a requirement. The cost of an MRI is around $800-1000 and insurance will not cover this examination for the purpose of diagnosing a leak in a cosmetic implant.

I recently saw an 80 year old man. Ten years ago, he had a squamous cell skin cancer on his scalp. His dermatologist recommended radiation therapy. Eight years later, the treated area broke down spontaneously and for two years he has had an ulcer on top of his head that has resisted all efforts to get it healed. On his examination now, he has a 2. 5 cm (1 inch) round ulcer on top of his head. The base of this ulcer is bare bone. The surrounding scalp is fibrous with no elasticity and clearly poor blood circulation. Getting this ulcer healed will require major surgery, if it can be done at all.

Dr. Bosshardt received his bachelor's degree in biology from the University of Miami, Magna Cum Laude, then attended the Leonard Miller School of Medicine of the University of Miami, graduating in 1978. He completed a general surgery residency at the United States Naval Hospital in Oakland, California. His residency term was interrupted from 1979-80 when he served as medical officer aboard the USS Wabash and was deployed to the Western Pacific with an aircraft carrier task force. During that year, the Wabash was involved in the rescue of Vietnamese boat people and was part of operations to end the Iranian hostage crisis in 1980. The USS Wabash crew received several decorations for this deployment, including the Meritorious Unit Commendation.

Liposuction
In theory, liposuction is pretty simple. Tiny incisions are made in the skin around an area of undesirable fat. Thin metal cannulas with small holes at the end are introduced into the fat, and the fat is then suctioned out. The procedure leaves tiny, nearly invisible scars. For liposuction to deliver on its potential as a cosmetic procedure, several conditions must be met.
Breast Augmentation
Breast augmentation is a procedure done to enlarge the breasts using implants. These consist of a shell, made of transparent silicone material, which is filled with either sterile saline solution or with silicone gel. Both can produce fantastic results, and the pros and cons of each will be discussed with you to select the best implant type.
Breast Reduction
Breast reduction surgery is a highly individualized procedure, and you should do it for yourself, not to fulfill someone else’s desires or to try to fit any ideal image.
Breast Lift
Loss of volume can only be recovered by replacing the breast tissue with an implant. However, some women prefer to go with what they have and just want the sagging corrected. This is where a breast lift, or mastopexy, can help turn back the clock a bit. In this procedure, markings are carefully made to outline the excessive skin to be removed and reshape the breast into a more youthful contour. Skin is then removed, leaving all the breast tissue to fill out the remaining skin as much as possible.

Breast reconstruction is a physically and emotionally rewarding procedure for a woman who has lost a breast due to cancer or other condition.

Neck Lift
Neck rejuvenation "comes in many sizes". For some, a simple course of Intense Pulsed Light (IPL) is all that is necessary to remove age spots. More aggressive topical lasers can reduce worsened pigmentation and wrinkles, as can a variety of chemical peels. LifeSculpt, our newest laser addition, can improve neck contour when loose skin accompanies extra fat in the neck. For those necks that need more, a "Neck Lift" can be performed. Neck lifts are known as "Cervicoplasties". A "Direct" Cervicoplasty simply removes the excess tissue from the central neck (the infamous "Neck Waddle" or "Turkey Neck") by cutting it out and closing directly, though this leaves a midline scar that really hides best in the male bearded skin. A "Full" Cervicoplasty leaves all scarring behind the ears, so scars are covered by hair and seldom seen. This procedure does require general anesthesia and still takes some time to fully recover from, but gives a nice neck tightening with minimal visible scarring. However, it cannot be done alone if the patient has large jowls, for then the jowls hang down on the tightened neck and the procedure doesn't give the improvement the patient needs. For the patient with jowls, a full Facelift must be performed. The Cervicoplasty is part of a routine Facelift.
Blepharoplasty
A blepharoplasty procedure is usually performed on adult men and women who have healthy facial tissue and muscles and have realistic goals for improvement of the upper and/or lower eyelids and surrounding area.
Abdominoplasty
An abdominoplasty, or tummy tuck, as it is commonly known, is an excellent procedure for improving the aesthetic appearance of the abdomen. A variety of problems can be addressed by this surgical procedure- stretch marks, loose skin, excess fat, and flabby muscles.
Breast Surgery
Many women seeking cosmetic breast surgery are not doing this in order to acquire larger breasts but are simply seeking to reverse some of the changes that come with time and the effects of pregnancy. These two factors are responsible for much, if not all, of the loss of firmness, volume, and "perkiness" of youthful breasts. Weight loss can also result in undesirable sagging of the breasts. In these cases, there is loss of elasticity of the skin, either due to the aging process, or due to the stretching of the breast skin from engorgement that accompanies pregnancy and milk production in the breast tissues.
