Emergency Care
The OrthoNOW! of Syracuse program treats non-emergency injuries like minor fractures, sprains, minor dislocations, and sports-related injuries. While these injuries may be serious, they do not require a trip to the emergency room. However, if you have a life-threatening injury, an open fracture, excessive bleeding, head trauma, or spine injuries, you should immediately proceed to your nearest emergency room.

Prostate Cancer
Predicting bone fracture in patients with metastatic disease. (PIs: TA Damron, KA Mann) Primary tumors, such as breast and prostate cancer, can metastasize to bone cause bone destruction and bone fracture. Predicting whether a bone with metastatic disease will fracture remains a clinical challenge. Clinical scoring systems based on X-ray and patient pain levels are not good predictors for determining which bones require surgical stabilization. We are using Finite Element (FE) modeling of clinical CT scan sets in collaboration with Dr. Timothy Damron to determine activities of daily living that are predictive of fracture.The long term goal is to use FE as a tool to help surgeons decide which patients to stabilize from those that are not at risk of fracture. Surgery was scheduled for May 12, 2017, at Community General Hospital. During those final three weeks with Vader 2.0, my arm got to be so bad that I was looking forward to the surgery. When I arrived at Community General on May 12, I met the team of doctors that would be assisting with the surgery. One of my anesthesiologists, Dr. Catania, had a great personality, kept things light, and let me watch on ultrasound as he injected my brachial-plexus nerve complex with a nerve block. I found the entire thing fascinating. In surgery, Dr. Harley harvested three inches of bone from my left iliac crest and dovetailed it to put it into my arm (basically carving it into a “T” shape that resembles a Tetris piece). He also put in a locking plate that was much larger than its predecessors along with 12 locking screws. I spent the night in the hospital figuring out how best to exist with Vader Arm 3.0 (also known as Vader Tetris) and Jetfire the Hip (so named after a Transformer who donates his parts to Optimus Prime, he can have a “power [he’s] never known”).
MRI
During the summer of 2015, I obeyed all of my medical restrictions, and my foot healed. In fact, when I had an X-ray in July 2015, Dan couldn’t see any sign of the original fracture and questioned whether the original X-ray and MRI had been read incorrectly (they hadn’t been read incorrectly; my compliance led to this fracture healing faster than he expected). Expecting to clear me 100% from this injury in early October 2015, Dan had me start off doing light weight-bearing activities (such as cycling and walking around the mall in my work boots to simulate what I would have to do at work), and he cleared me to run again starting on September 18, 2015, when my custom orthotics arrived. I ran one mile that day and then went on a bike ride with friends. I never made it back from that ride; I crashed and sustained an open, compound fracture to the radius and ulna of my left arm.

X-Rays
Computed Tomography
Traditional X-rays use a stationary X-ray machine to create two-dimensional images of a specific area of the body, much like a photograph. During a CT scan or computerized tomography, an X-ray unit rotates around your body and takes cross-sectional 3-D pictures, like slices, of the inside of your body. A computer then integrates these images to show bones and soft tissues with greater detail and clarity than regular X-rays. CT scans are used to visualize injuries to bones and surrounding soft tissues, such as tendons, ligaments, and muscles.
Chemotherapy
Musculoskeletal sarcomas are a family of cancers that affect the connective tissues, and are treated by a specially trained orthopedic oncologist. Several types of sarcoma, including osteosarcoma, rhabdomyosarcoma and Ewing Sarcoma, are particularly prevalent in children and adolescents, and require multimodality treatment regimes that may include surgery, chemotherapy and radiotherapy. Even with the most aggressive treatment plans, these childhood cancers can be fatal in up to 84% of cases, depending on the type, location and stage of disease. Furthermore, the best available treatment strategies can result in lifelong toxicity syndromes affecting the quality of life in survivorship. By focusing on the unique molecular characteristics of these pediatric cancers, our research group seeks to develop more effective and less toxic treatment strategies that will improve the duration and lifelong quality of survivorship for these children. Critical to this goal is the development of animal models as a disease-relevant platform for testing of new treatment strategies. This research has been supported by grants from the National Cancer Institute, the National Institute of Arthritis and Musculoskeletal and Skin Diseases, the Children’s Miracle Network, Jim and Juli Boeheim Foundation and the Page Trucking Foundation.
Radiation Therapy
One in eight women will develop breast cancer at some point in their life. The majority of women with advanced breast cancer develop secondary tumors (metastases) in their bones. Although these bone metastases are not usually life threatening, they cause significant pain, restrict the patient’s ability to carry out activities of daily living, and weaken the bone to the point where spontaneous fractures (known as “pathological fractures”) may occur. In 1995, Matthew Allen’s mother, Kate, was diagnosed with an aggressive form of breast cancer that subsequently spread to bone. This personal experience led to the development of a research effort aimed at better understanding the interactions between breast cancer and bone with the goal developing improved therapies for patients with bone metastases. An animal model was developed that mimicked many of the key features of bone metastasis, including localized bone destruction and increased bone fragility. Treatment with radiation therapy was shown to be capable of killing tumor but incapable of preventing bone fragility. We successfully used bisphosponates (drugs that are widely used to treat osteoporosis in women) and anabolic agents (drugs that build new bone) to reduce the risk of pathological fracture in the mouse model. We developed a computer modeling approach to predict the risk of pathological in mouse bone, with the long-term goal of translating this into clinical use in patients with bone metastases. This work has been extended to a cohort of human patients where we have validated rigidity analysis methods and developed a finite element approach to predict fracture risk in breast cancer patients.

Knee Replacement
® is a viscosupplementation injection consisting of hyaluronic acid, which is used to treat osteoarthritis knee pain. Each injection lubricates and cushions the knee, providing pain relief for up to six months. Viscosupplementation injections are typically given weekly for 3 or 4 weeks. This type of injection is the desired treatment for people with knee arthritis that is refractory (unresponsive) to more conservative therapy. It is useful for people who are trying to delay invasive surgery or are not suitable for knee replacement surgery.

Dan told me that the risk of displacing this fracture was high, and that if I did displace it, I would need reconstructive surgery on my foot that involved removing the sesamoid bone (and therefore never be able to run again). However, twelve weeks after surgery, Dr. Azer determined that my ulna was not healing. My radius was making progress (albeit slowly), but my ulna remained unchanged since September. I was also having issues with pronation and with nerve pain; Dr. Azer told me that he was at his limit for how he could help me and that he would have to refer me to one of his colleagues. He referred me to Dr. Joshua Pletka, who took over my care at the end of December 2015. Dr. Pletka diagnosed my injury as a non-union fracture in March 2016, and after I had a nerve conduction test done in that same month, I was also diagnosed with lesions on my left median and anterior interosseous nerves. He told me that I would need surgery to attempt to fix the non-union fracture. We had a candid conversation about what that would mean, and he told me that it was not medically necessary for me to have surgery immediately.
