Sinusitis
Interestingly, unlike other white blood cells, eosinophils seem to have a primary function of killing macro organisms such as worms and fungi. Thus, fungi also attract eosinophils and appear to play an important role in chronic rhinosinusitis. Non-invasive fungal rhinosinusitis affects multiple sinuses in young adults. Treatment may include anti-fungal medication, oral steroids, intranasal steroids, oral decongestants, mucolytics and immunotherapy. Functional endoscopic sinus surgery is also a treatment for the management of fungal sinusitis. If there is suspicion of a fungal infection or if your cultures are positive for fungus, you will be treated accordingly.

Emergency Care
Blood from the nose is called epistaxis and can be divided into two types. These are anterior bleeds and posterior bleeds. The anterior nose bleeds arise towards the front of the nose and typically seen on the septum in an area called Killsebach’s plexus (aka Little’s area) or on the soft tissue prominence across from the septum called the inferior turbinate. In otherwise healthy individuals, anterior epistaxis is typically can be controlled at home, the less common posterior epistaxis usually requires a visit to either the emergency room or to you ears, nose throat surgeon.
Immunizations
The influenza immunization plays an important role in medical therapy. This is especially true for groups at high risk for influenza-related complications. This includes persons greater than 55 years old, residents of nursing homes or long-term care facilities, people with chronic disorders of the pulmonary or cardiovascular system, and patients with chronic metabolic diseases, renal dysfunction, hemoglobinopathies or immunosuppression. The best time to have the flu shot is October to mid-November before the start of the influenza season.

Endoscopy
Nasal disorders are traditionally diagnosed through careful history and physical examination. However, sinonasal symptoms can provide limited information for diagnosis. Thus, objective information is sought through physical examination. Although there are many methods available to directly evaluate the nasal passages, three are typically employed for most patients with long standing sinus or nasal complaints. They are: anterior rhinoscopy (looking into the nose with a headlight and nasal speculum to open nostrils), nasal endoscopy (looking into the nasal passages with an endoscope or telescope) and CT scanning. These techniques reveal important information necessary for proper diagnosis, treatment and monitoring. Often times, nasal endoscopic findings compliment findings seen on CT imaging, but they are not always the same. Interestingly, abnormalities detected on endoscopy or imaging can be present without symptoms.

Minimally Invasive Surgery

Otolaryngology
Dr. Solyar completed her Rhinology & Fellowship training at the Sinus& Nasal Institute of Florida in 2011. Subsequent to her fellowship at SNI, Dr. Solyar returned to the Northeast to practice otolaryngology for 1 year before returning to SNI.
