Step 2: Care Plan Using the information gathered in the assessment, our team of care consultants develops a plan of care that outlines problem areas, establishes goals and provides resources to meet those goals. The care plan helps you or your loved one navigate through the health care system with continued re-assessment to meet the client's needs. Step 3: Implementation The discussed and agreed care plan becomes a reality as we balance and coordinate resources of the team and the needs of the client. Your care consultant coordinates all the actions of the team and provides feedback to the client. Your care consultant's sole responsibility at all times, are the client's needs.
