Pediatric Care
She has completed training in alternative therapies in adult and family practice. Courtney has previous experience within the Adult Progressive Care Unit, Pediatric/Neonatal Intensive Care Unit and Pediatrics. Additional patient care populations include Geriatric Psychiatry and Subacute Adult Rehabilitation.
Primary Care
Dr. Cushing has specialized training in forensic (court ordered) evaluations, childhood disorders (e.g., ADHD, learning disabilities, bipolar, etc.), addictions, and neuropsychological assessment. He has performed thousands of evaluations on children, adolescents, and adults for numerous referral sources. He regularly works in tandem with psychiatrists, primary care physicians, pediatricians, attorneys, and teachers to insure accurate diagnosis and specific treatment recommendations for his patients. He has hospital privileges at Rockford Memorial Hospital.
Family Practice
Courtney Forman is a dual certified Family Nurse Practitioner (FNP) as well as Psychiatric Mental Health Nurse Practitioner (PMHNP). She began working with In Home Medical Group (IHMG) in 2016. She obtained her Bachelor of Science in Nursing degree with a minor in Intercultural Studies from Indiana Wesleyan University in 2011. Courtney then went to get her Master of Science in Nursing degree with a focus on family practice from Northern Illinois University with FNP certification through the American Academy of Nurse Practitioners. In 2021 she completed a post master's certificate at University of St. Francis and completed PMHNP certification through the American Nursing Credentialing Center.
Immunizations
In the Rockford Community Elise served as a Trustee of the Community Foundation of Northern Illinois and served as Board Chair in 2012-2013. She has served as a member of the Head Start Medical Advisory Board, the Carrie Lynn Center Consultation Team, and is past Chair of the Dean's Advisory Council of the University of Illinois, Rockford. With over 30 years of clinical experience, Elise has served on the boards of Big Brothers/Big Sisters, Children’s Development Center, Rockford Area Substance Abuse Council, Rockford College Board of Counselors, United Way of Rock River Valley, Rockford Womans’ Club, and the University of Illinois Chancellors Committee on the Status of Women. While a member of the Junior League of Rockford, she participated in the development of Youth Services Network. Elise joined the Rockford Rotary Club in 1988 and served as their first woman President (1999-00). In 2009-10, she served as District Governor for the 49 Rotary Clubs of Northern Illinois. Since 2007, Elise has led Rotary service teams to Nigeria and Uganda for project development and End Polio Now immunizations. In 2013, Elise received the Rotary International Service Above Self award and in 2018 was a recipient of Crusader Clinics' Spirit of Caring award. Elise remains committed to providing quality mental health services to children, adolescents and adults.

Autism
Michelle enjoys working with adolescents, adults and families. She is experienced with an array of diagnoses including anxiety, depression, adjustment disorder, ADHD, autism spectrum disorder, and end of life issues in adults. Michelle believes in a solution-focused therapy where the therapist and individual walk hand in hand to find solutions for the complicated maze that is life. Many treatment modalities are utilized from Cognitive Behavior Therapy to Mindfulness Based Stress Reduction. Other therapies are intertwined such as Dialectical Behavioral Therapy and Acceptance and Commitment Therapy. Michelle has extensive post graduate experience with individuals who have intellectual and developmental disabilities as well as their family and/or caretaker. Michelle believes that working with the supports around the individual is as vital as working with the individual themselves.

Attention Deficit Hyperactivity Disorder (ADHD)
Lisa enjoys working with children and adults. She works with a variety of diagnoses but specializes in behavior disorders, ADHD, autism spectrum disorder as well as anxiety, depression, grief, death/dying and end of life issues in adults. Lisa enjoys helping families build more effective skills for building and resolving ineffective communication. Many are attracted to Lisaâ€™s gentle yet enthusiastic spirit in their journey toward healing. Lisa combines play, sand work, cognitive behavioral therapy and brief solution focused therapy to help people live better lives.
Stress Management
Angela has been a long-standing member of the National Association of Social Workers and has held board positions with the local chapter throughout the years. She is the Clinical Director of the Blackhawk Region Critical Incident Stress Management Team and assists in providing stress management services to first responders in the Northern Illinois region after a critical incident has occurred. She is also a board member of the Junior League of Rockford (JLR) and has been actively involved in community projects that work toward the betterment of the Rockford community for the past several years.

Dr. Kyle Cushing is a Licensed Clinical Psychologist and Consulting Forensic Examiner. He was raised in Rockford and has earned a Bachelor of Science degree in psychology from the University of Iowa, a Master of Science degree in applied psychology from Jacksonville State University, and a doctorate in clinical psychology from the Illinois School of Professional Psychology. His one year APA (American Psychological Association) accredited internship was completed through the U.S. Federal Bureau of Prisons at the Federal Medical Center in Fort Worth, Texas. He was also an intern psychologist for the Fort Worth Public School District. He then completed a one year post-doctoral fellowship at the Center for Forensic Psychiatry in Ann Arbor, Michigan, before returning to Rockford where he has been in private practice since 1999.

Daniel has provided mental health counseling in many different capacities over the years including: counselor at a substance abuse treatment center, in-home therapist for families involved with the Department of Child Services, psychotherapist at a behavioral health clinic, and mental health counselor for students at a university counseling center. Daniel has extensive experience working with individuals and families who struggle with various mental health issues including: stress & anxiety, depressive episodes, traumatic experiences, loss of a loved-one, discordant dynamics in couples & families, and difficult life transitions.
