Plantar Fasciitis
Contributing factors of plantar fasciitis can also be from tightness on the calf muscle. Extracorporeal shockwave therapy (ESWT) is an advanced new technology used to treat several different conditions within the foot, such as plantar fasciitis and heel spurs. Originally used to treat kidney stones and other urologic conditions, ESWT is now offered as an alternative treatment for orthopedic conditions in patients who opt out of surgery.
Endoscopy
Insertional heel pain with or without the spur is most commonly related to mechanical factors.1-4 Other proposed causes of heel pain include bursitis, periostitis, nerve entrapment, trauma, degeneration of the plantar heel fat-pad, and seronegative arthridities.5-1[1]Up to 90% of patients can receive complete relief of symptoms with conservative therapy alone (Table 1).1-9, 11-31 When conservative methods, including extracorporeal shockwave therapy, fail to alleviate heel pain, surgical intervention may be indicated. The most widely used surgical technique used today was popularized by DuVries.32 Variations of this procedure are based on the location of the incision and the amount of fascia and bone that is resected (Table 2;Fig. 1).9, 16, 17, 32-37 Alternative surgical techniques include endoscopy and coblation.3, 8, 38 The endoscopic plantar fascia release technique has been reported to be an effective alternative to relieving plantar heel pain.3 The coblation technique for repair of inflamed and degenerated tendon, ligament, and fascia offers promise for treatment of the plantar fascia, but longterm studies are needed.39 Advancement in diagnostic technology has enabled the ultrasound to become a reliable, cost-effective, diagnostic, and therapeutic instrument for visualizing foot pathology.40-42 These advancements have sparked interest in the use of ultrasound to assist the surgeon in the management of plantar fascia pain (Figs. 2 and 3). The rationale for the technique is based in part on a previous ultra-sound study that positively correlated the thickness of the medial, central, and lateral plantar fascia bands with heel pain (Table 3).2 Hence, the purpose of this ultrasound study is to determine the effectiveness of the ultrasound-guided plantar release in relieving heel pain.

Neurolysis
Ultrasound-guided plantar fascia release offers the surgeon visualization of the local anatomy at the surgical site and enables more precise anatomical dissection. In addition, there is minimal tissue trauma to the adjacent anatomy surrounding the plantar fascia because of the small size of the dissecting instruments. In the present review, there were no postoperative complications of infection, hypoesthesia, or sharp heel pain. However, some complications have been reported by other authors. Lester and Buchanan45 noted that 42% of patients had heel hypoesthesia postoperatively. Their procedure consisted of spur resection, release of all first-layer plantar musculature, and fascial release. Such extensive dissection may have resulted in transaction of the abductor digiti quinti muscle and the calcaneal nerve branches. Baxter and Thigpen46 report 18% of patients continued to have heel pain with weightbearing after open neurolysis procedures were performed. Leach et al47 performed open fascial release, resection of the fascia, or both

Bone fractures can be diagnosed by physical examination and an X-ray or CT scan. Immobilizing the area is often helpful in relieving pain before proper treatment begins. Treatment for bone fractures depends on the location and type of fracture, as well as the patient’s medical history. We take all of these factors into account when developing a treatment plan.
Iontophoresis
This study was undertaken to determine the effectiveness of acetic acid iontophoresis in the treatment of heel pain. Thirty-five patients with chronic heel pain were treated with acetic acid iontophoresis over a 4-year period. Ninety-four percent of patients had complete or substantial relief of heel pain after an average of 5.7 sessions of acetic acid iontophoresis over an average period of 2.8 weeks. Heel pain levels were rated from 0 to 10, with 10 representing the most severe pain. Heel pain prior to iontophoresis treatment received an average rating of 7.5; by the end of therapy, the average rating had decreased to 1.8. At an average follow-up time of 27 months, heel pain levels averaged 0.64, indicating continued reduction in heel pain. Ninety-four percent of participants said that they would recommend acetic acid iontophoresis to someone with similar heel pain.

Arthroscopy
Surgical instruments will then be inserted if needed through additional incisions to remove or repair tissue within the joint. The instruments and arthroscope are then removed and the incisions are closed with sutures. The arthroscopy procedure usually takes 30 to 45 minutes to perform.

Bunion Surgery
Recovery from bunion surgery depends on the complexity of the procedure. Some people will be able to walk on their foot immediately after surgery in a special shoe, while others may need to use crutches or a cane and may not be able to walk for a few weeks or longer. It is important to keep your dressing clean and dry to ensure proper healing. Most patients require the use of orthotics and physical therapy after surgery in order to maintain stable and correctly-positioned feet.
Bunions
Bunions can form when there is an improper balance of forces exerted on the joints of the foot, causing instability in the joint of the big toe. This often occurs as a result of shoes that do not fit properly, abnormal walking habits, or an inherited foot type. Bunions can also be caused by injury, birth defects, arthritis, or certain neuromuscular disorders.
Heel Surgery
Five patients with bilateral plantar fasciitis and 36 with unilateral plantar fasciitis were included in this review. A diagnosis of plantar fasciitis was initially based on the patientâ€™s history of poststatic dyskinesia and physical findings of pain on palpation of the plantar fascia at its calcaneal insertion. Patients were excluded if they had any of the following: 1) a history of inflammatory arthritis, 2) a connective-tissue disorder, 3) recent trauma to the heel or plantar fascia, 4) a congenital defect of the lower extremity, 5) previous heel surgery, 6) nonsteroidal anti-inflammatory medication use within 2 weeks, or 7) a corticosteroid heel injection within the previous 2 weeks. If the plantar fascia pain was not alleviated conservatively, ultrasound examination was performed in the office to locate symptomatic and asymptomatic plantar fascia bands. The same examination was also performed prior to anesthesia with a linear 7.0-MHz ultrasound transducer (Accustom128XT; Acuson Corp, Mountain View, California). Ultrasound examinations were performed while the patients were positioned supine with their feet hanging over the edge of the examination table. A bead of acoustic gel was applied to the cover of the head of the transducer, which was then placed longitudinally on the plantar aspect of the foot (Fig. 4). Ultrasound scanning was performed during dynamic dorsiflexion of the toes to stretch the plantar fascia, allowing its margins to be delineated. Images were recorded on emulsion film with a multi-image camera (Fuji Corp, Hyogo-ken, Japan). Transverse sonograms were also obtained to determine the location of the symptomatic plantar fascia bands proximally near insertion into the calcaneus (Figs. 5â€“7).
Arthroscopy
A retrospective analysis was conducted of 32 patients (33 ankles) who has undergone surgical ankle arthroscopy for chronic ankle pain that was recalcitrant to conservative treatment. All patients were examined clinically and completed a written questionnaire Intraoperative ankle arthroscopy showed hypertrophic synovitis, adhesive bands, chondral bands with synovitis, osteophytes, and abnormalities in the talar dome. Results in treatment after and average follow-up time of 1.4 years showed ankle scores of 15 excellent, 11 good, 5 fair, and 2 poor.
Ingrown Toenails
Some ingrown toenails can be treated at home by soaking the foot in warm water, applying antibiotic cream to the area and wrapping the toe in gauze or bandages. It is important to keep the area clean to help prevent infection, sometimes infection can occur very quickly if not treated correctly. If you see redness or swelling see Dr. Vohra immediately to have your foot assessed. Your doctor may cut under the nail to separate it from the skin that it is growing into, encouraging growth above the edge of the skin. For more severe or recurrent cases, part of the nail and the underlying tissue may be removed in order to remove the infection. Removal can be done through chemical, laser, or other methods. Patients can prevent ingrown toenails by protecting the foot from trauma and by wearing comfortable, well-fitting shoes. Patients with diabetes may be at a higher risk for infection or other complications of ingrown toenails, and should take special precautions to prevent them from occurring.

A bunion is a common condition that involves an abnormal, bony bump at the base of the big toe, causing the joint to swell outward and become painful. The big toe may also turn inward toward the second toe as a result of the enlarged joint, which can then lead to difficulty walking, ingrown toenails, and corns and calluses.

Toenail fungal infections, also known as onychomycosis, affect an estimated 40 million people in the US as a result of genetics, aging, trauma, or poor foot care. Patients with toenail fungus may experience thick, brittle, and discolored nails that may split. If left untreated, an infection may lead to difficulty wearing shoes and walking.
Lesions
Treatment for nail diseases may include oral or topical medications. The nail may need to be removed for severe infections. Patients can prevent nail conditions from developing by keeping their feet clean and dry, wearing shoes that fit well, and clipping toenails straight across. Sports medicine is a specialty that deals with the prevention, diagnosis, treatment, and rehabilitation of injuries suffered during athletic activity. The goal of treatment is to heal and rehabilitate the injury so patients can return to their favorite athletic activities quickly, whether it’s Little League, recreational play or high school athletics, college or professional sport.

A physical examination and one or more imaging tests will determine whether the pain is caused by a neuroma or by a condition with similar symptoms such as arthritis, stress fractures, tendon inflammation, tarsal tunnel syndrome, or from nerve compressions in the ankle or leg. Fungus Toenails
While many other treatments are available to treat toenail fungus, many are not as effective, safe, or minimally invasive as laser treatments. Medications often do not fully remove all traces of fungus and frequently result in recurrence, while surgery must be combined with oral therapy in order to thoroughly treat and remove the infection.
Hammertoes
A hammertoe is a term used to describe a crooked, deviated, or contracted toe. Although the condition usually stems from muscle imbalance, it is often aggravated by poor-fitting shoes or socks that cramp the toes. Over a period of years, the tendons that move the toe up and down begin to pull the toe with unequal tension, and the toe then begins to buckle or become contracted, causing an abnormal “v”-shaped bending of the little toes. Patients with this condition often experience pain, swelling, redness and stiffness in the affected toes.
Heel Spurs
A comparative retrospective study of 48 open heel spur surgeries and 20 endoscopic plantar fasciotomies was conducted involving 59 patients over a 10-year period. There was a significant reduction in heel pain at the time of follow-up (average, 3 years) for both groups. Overall, 85% of procedures were associated with patient satisfaction with the results, and patients said that they would recommend heel spur surgery for relief of severe heel pain in 94% of cases. Factors influencing the postoperative outcome, such as duration of preoperative symptoms, extent of conservative care, and obesity, are discussed.

An ingrown toenail is a common condition that involves the corner of the toenail growing into the skin of the toe, causing pain, swelling and infection.
