Grand Oaks Health and Rehabilitation Center is a place to get better, a place for living. We are a 120-bed skilled nursing center, and a proud member of the Consulate Health Care family. Opened in 2000, we have 20 private rooms, 50 semi-private rooms, and can provide accommodations for couples that become residents of our care center. Grand Oaks also offers features such as an outdoor recreation area, garden and patio, a rehab gym, a beauty/barber shop, a library, pastoral support and transportation services. Residents and often time's family too, can enjoy planned activities such as excursions, various arts and crafts, gardening, group movies, discussion groups, visiting performers, and intergenerational programs. After reading former statements from others. I was ready to see a real hell hole.
My companion was transferred from the hospital almost a week ago, and she hasn't had any major complaints. Au contrere, she has good to say about how she is being treated and the interest in the rehab exercises, she has received.
The food is "hospital" food, is about the only complaint, so far.
My friend thinks the facility is well staffed and kept very clean.

my mom (85) has been at Grand Oaks for 4 months after a bad leg fracture. when she first arrived i would agree with some other reviewers, her roommate was loud and prone to episodes of throwing things, the day nurse was very condescending and the aides didn't seem to care much at all (every request was "an effort"). HOWEVER, after a room change, she had the best nurse ever (Deanna). ( i've been a nurse 40 yrs.. i know). this woman was on top of everything, healed up a lingering wound, addressed every concern promptly, was caring/supportive. Rehab PT (Marta) is OFF THE CHARTS good. she is just amazing!!!, always upbeat, encouraging..just goes that extra mile day after day. Mom had a couple physical set backs during her recovery and if it wasn't for the support she received at Grand Oaks things would not have gone as well as they did. Also have praise for house cleaning, dietary and administration that addressed issues quickly and efficiently. i would highly recommend Grand Oaks.

Worse place to take a love one they're always short-staffed the CNAs and the RN are rude. At night the staff are Extremely Loud and do not care if the patients are sleeping or not. My mother was left in pain for 24 hours before her meds arrived. CNA and another staff member from housekeeping came into my mother's room and we're talking about my mother's diagnosis my mother felt ashamed and humiliated when I made a report the administrator Donna Smith did not even care. To top everything off my mother was placed with a roommate that was aggressive and combative this patient did not belong in this facility yet they took her to fill their beds. When my mother was signing the admission agreement from a dawn adam she kept calling my mother by a different name she had someone else's records. This place staff members starting from the administrator to the cna they do not know there a$$ from there elbow. I will never go back and I will definitely not even bring my dog's there!

How can you call yourself a rehabilitation center?? The nurses there could care less about the patients. I have been there several times and all their doing is talking amongst themselves. I was there visiting a loved one and the man in the same rooms bell was going off and nobody came in. I went to the nurses station and had to request that somebody come in and help him. Two of the nurses gave me a dirty look (like why did I care) and kept on talking. One nurses name was Tina. It's absolutely HORRIFYING, to know that these so called nurses still have a job at this facility after some of the things I have seen. The lady across the hall had not been taken care of for hours. They just don't care at this place. I intend to write a letter to our governor of Florida in hopes that he steps in and does something because Grand Oaks cannot rehabilitate a single soul that unfortunately has to be in there. It's not that they cannot, it's that they will not. They refuse to care.

WARNING: DO NOT PUT YOUR FAMILY MEMBER IN THIS PLACE. The staff is very pleasant to your face but treat the patient's rudely and with disrespect. We brought my mother-in-law home yesterday and she is a totally different person. She's scared, angry, and distrustful; the total opposite of what she was before entering this horror of a healthcare facility. If you file a complaint, they meet with you and tell you they will get down to the bottom of things but its all lip service. If they say they will get back to you...forget it. You'll never recieve a call or resolution. Ask too many questions? They will find a way to expidite the discharge of your family member. Please DO NOT trust this institution with your loved ones.

Be Aware

Have a personal friend there which she always lucid. Went to see before 8pm and she was in a daze and confused and seeing people that was not there. looked for the nurse in all her rooms she was not to be found had to go to there welcome center to get her paged. She was not there for over 20 min. When she arrived she denied given medication and it was so obious that she must had given a sleeping pill and a strong one also sad part looked like most of thos patients at night must have taken the same pill. By 8:30 most of them was asleep and then a machine to clean the floors was passing by and made so much noise. A nurse came in at 10:08 pm 12/16/14 my wife said shhhh cause she was talking so loud to the nurse station and she turned to my wife said you do not shhh me i have a patient no way she was curtious as she left she slamed the bathroom door and left about 10 min later she came back to open the bathroom door and left it open how strange. I am glad we are taking her out today. At first many said to me ( patients ) its an evil place its nice when family members are there but when there not they change well i witness this and i could not sleep last night thinking about this. I hope my review will help this facilty to be aware and give a true meaning of care. I will do my best to get this out and hope it gets investigated on how they really treat there patients at night. This is a shame to the community of Palm Coast i urge all that have family please visit them more often if you do not they do what ever they want to them there. Now during the day when i was there they where nice and pleasant but this a pure case of dr. Jekal amd mr hyde.
