The Middletown Community Health Center is a full-service diagnostic and treatment center with more than five locations throughout the state of New York. It is a nonprofit, community-based organization that serves more than 12,000 patients. The center has a staff of over 25 employees. The Middletown Community Health Center participates in the Health Disparities Collaborative through the United States Department of Health and Human Resources. The center offers a range of specialty, primary care, nephrology, gynecological, social work, and HIV and nutrition counseling services.
