Dental Examination

While current therapy does not allow for a cure to gum disease, professional intervention is critical to prevent its progress and maintain health.
X-rays
Computed Tomography (CT scanning) has been available for medical diagnosis for an extended period of time. Cone Beam technology has allowed the development of similar devices in dentistry with much lower radiation exposure (similar or less than routine dental x-rays) and very precise imaging. This is performed in the office and the results are available within minutes allowing for the most accurate diagnosis and treatment planning. In addition, where appropriate, virtual surgery (implant placement) can be used and computer generated guides can be fabricated to allow for extremely precise outcomes.

Teeth Cleaning
Periodontitis is an inflammatory disease of the gums and bone that result from bacterial accumulation in certain genetically predisposed individuals.Â While it cannot be cured, once it is under control continued professional maintenance is critical.Â This often occurs more frequently (every 3-4 months) than typical dental cleanings. The treatment is minimally invasive using standard cleaning instruments, but may involve isolated areas of scaling and root planing with possible local antibiotics where inflammation and deeper pockets persist.Â Alternating these maintenance visits with your dentist is critical to insure the co-management of your continuing collective dental needs.

Periodontal Plastic Surgery is directed at improving the health and/or the esthetic appearance of your smile. With crown lengthening, Drs. Harris and Poth can even out a gumline to create a more radiant smile as well as expose decayed or fracture teeth to allow for proper restoration. Exposed roots of teeth can be covered with minimally invasive gum grafting. These procedures are a predictable way to cover unsightly, sensitive roots and to prevent future gum recession.

Gingivectomy and Gingivoplasty
These procedures are directed at improving the overall esthetic appearance of your smile. They can include: gingival grafting, gingivectomy, and esthetic crown lengthening. With esthetic crown lengthening, a gummy smile can be altered to expose more tooth. Exposed roots can result from irritation, inflammation or aggressive brushing and may be covered through gingival grafting procedures.
Periodontal Surgery
Uncontrolled periodontal disease has been shown to increase CRP levels with treatment resulting in a documented improvement (reduction in CRP). In addition, periodontal bacteria have been isolated in coronary arteries removed during coronary bypass surgery. Further reports have even shown a widening (improvement) of the lumen (open area through which blood flows) of the coronary arteries of patients with more advanced chronic periodontal diseases following periodontal surgery.

Osseous Surgery
Many times, the early stages of periodontal disease are best treated with non-surgical periodontal therapy. Even in severe cases, non-surgical periodontal therapy often precedes surgical therapy. This is done to improve the overall tissue quality prior to surgery and also to help limit or eliminate the areas requiring surgery. We pride ourselves on the fact that we are very conservative in our treatment recommendations and limit surgery to the areas where it is absolutely necessary.Â PERIODONTAL SURGERY; OSSEOUS SURGERY; GUIDED TISSUE REGENERATION; LASER PERIODONTAL THERAPY.
Tooth Extractions
In certain situations implants can be placed at the time of tooth extraction. Bone grafting may be necessary to preserve tissues around implants or rebuild lost tissues prior to implant placement due to long-term tooth absence or trauma. Fortunately, once implant osseointegration has occurred, normal masticatory function (chewing) stimulates the bone and helps it remain viable and healthy. Daily home care is critical for implant health and professional attention to these areas occurs at routine maintenance visits.
Ridge Augmentation
The loss or need to remove one or more teeth can occur for a variety of reasons.Â Ideally efforts to save teeth are considered first, but should be employed when the prognosis (odds of an outcome) to achieve this is fair to good.Â If a tooth cannot be treated or the effort will not render a long-termÂ success then removal and replacement of teeth can be achieved using dental implants.Â As with all diseases and conditions, early diagnosis and planning is critical.Â Before a tooth is removed its current and/or future role in chewing, smiling and speaking should be considered.Â If replacing a tooth or teeth is important to maintainÂ or regain any of these roles, then a plan to achieve this should be in place before the extraction occurs.Â However, if a tooth is already missing we can determine what needs to be done for its successful replacement.Â This may include rebuilding bone or modifying the position of sinuses in select circumstances.Â BONE GRAFTING; RIDGE PRESERVATION; ALVEOLAR RIDGE AUGMENTATION; SINUS ELEVATIONÂ
Bone Grafting
In either case, the skin membrane of the sinus is gently elevated (lifted) upwards to create more space for implants. This new space is supported by the placement of a bone graft which is done either concurrent with implant placement (8-12 weeks of maturation) or independently prior to implant placement (approximately 4-6 months of maturation). As with the other bone grafting procedures, this new bone becomes your own and responds favorably to implant placement and restoration.

Â Dr. Poth has vast teaching experience mentoring over 80 Advanced Education in General Dentistry residents in all aspects of Periodontics during his time with the Air Force. Â He continues to further his education in part through the Western Ohio Seattle Study Club (a Seattle Study Club Affiliate) as he strives to deliver quality outcomes for his patients.
