Nutritional Counseling
We recognize the importance of proper nutrition as a key element of good health, and the impact it can have on chronic diseases such as diabetes, hypertension, and high cholesterol. All Forward Focus patients have access to our on-site Licensed Dietitian Nutritionist, Sheryl Gray, who collaborates with Dr. Berliant, Dr. Katz, Dr. Lasin, and Dr. Schaffel to develop a customized nutritional plan that improves overall health. Our personable, motivating dietitian will work with you to understand your lifestyle, eating habits, health issues, personal goals, and design a realistic, actionable plan that will work best for you. Sheryl uses a three-tier approach. The Nutrition component promotes a non-diet approach with an emphasis on healthy food choices & portion control. The Behavior Modification piece evaluates existing behaviors & offers strategies to overcome triggers for poor choices & to improve self-esteem. The Physical Fitness portion focuses on improving physical fitness by moving more using a variety of different exercise techniques. Plus, we will communicate your progress directly to your doctor, providing a seamless path to wellness care.
Primary Care
During my residency training, I was fortunate enough to take care of a low socioeconomic patient class which often had limited medical access and therefore presented with a wide variety of late disease manifestations. After completing my residency, I began working in the inpatient setting as a hospitalist within the Northwestern System and cared for some of the most medically complex and sickest patients. I have witnessed the depths to which disease can manifest itself and have come to understand that so much of what I observed could be prevented. My passion for primary care and prevention has been cultivated by these experiences.

If or when your illness requires hospitalization, Dr. Schaffel,Â Dr. Lasin, Dr. Berliant, and Dr. Katz will visit you at Northwestern Lake Forest Hospital or Highland Park Hospital, consulting with specialists and nursing staff to advocate on your behalf to ensure you receive the care you deserve. They are able to meet most patients in the emergency room during critical situations. If you are hospitalized at another institution, your physician will consult with specialists by phone and advocate for the highest quality, coordinated care for you.
Electrocardiogram
Yes, the annual fee covers your annual exam and standard lab testing and an EKG that is a part of this exam. However, any additional special testing, necessary injections or vaccines, x-rays etc. outside of the standard lab testing will likely be covered by your insurance. There is no office visit fee for this exam.
Immunizations
After a thorough physical exam, Dr. Schaffel, Dr. Lasin, Dr. Berliant, or Dr. Katz will discuss your history, screening tests, any x-ray results, vaccination records, and medication management, as well as any medical concerns and additional life factors that may be impacting your health.Â

He attended the University of Michigan as an undergraduate, and then received his Doctor of Medicine (MD) at the Chicago Medical School - Rosalind Franklin University. He completed his residency in internal medicine at Rush University Medical Center, where he was also honored with a Resident Teaching award. He is board certified in Internal Medicine.

We are delighted to have Paul Later; MD participate in our 2023 Prevention & Wellness Education Series.Â Dr. Later was raised in the Chicago area and went to high school at the University of Chicago High School. He went to college at the University of Chicago and then medical school at Rush.Â He did his internship at Northwestern then went back to Rush for his neurology residency and a neuromuscular and electrophysiology fellowship.Â He moved to Fort Wayne, Indiana for more than 20 years, getting deeply involved in a number of different subspecialty areas within neurology.Â He was involved in clinical trials in stroke, migraine, dementia, and MS.Â He moved back to the Chicago area and was at Northwest Community Hospital for 2 1/2 years as stroke co-director and while there, was involved in enrolling patients into the IDEAS study which was a nationwide study of amyloid PET scans for patients with mild cognitive impairment or early dementia to better identify patients with questions remaining about their diagnosis whether there is Alzheimerâ€™s pathology demonstrated on the PET scan.Â

Stress Management
To me, taking care of patients begins with an understanding that the foundation of wellness is defined by four pillars of health which are: nutrition, exercise, sleep, and stress management. I firmly believe that a large percentage of the chronic ailments that plague our society could be prevented and often even reversed if the above areas are optimized. While there is always a role for prescribed medication, and I certainly have no qualms about prescribing evidence-based therapies, my goal is to minimize the drugs being prescribed and instead focus on empowering patients to be their own vehicle of change by slowly improving their lifestyle.Â The power of a healthy diet combined with movement cannot be overstated. Nor can treating sleep and stress management be overlooked. In my opinion, these strategies will almost always be more impactful than a prescribed pill. The goal isnâ€™t just to prolong an individualâ€™s lifespan, but perhaps more importantly their healthspan. In other words, helping patients live longer, but with the strength, vigor, and cognition to continue doing all of the things that they love until late in life. In my own busy life, I try to adhere to the same values that I discuss so frequently with my patients. Rest assured, if Iâ€™m recommending a lifestyle intervention to my patients, itâ€™s only after I have carefully vetted the prescribed strategy and worked towards optimizing this aspect of my own life. I believe that anyone can learn to enjoy healthy food and exercise, itâ€™s only a matter of experimenting till you find what works for you. Like anyone, Iâ€™m not perfect at following my advice, but I can certainly say that I feel my best when I do. I am eager to continue helping my patients on their own journey towards a healthier life.

Lisa Lewis brings a wealth of experience in marketing and customer service to Forward Focus. She has joined our administrative team to enhance the communication and coordination of your care. Lisa also has a strong interest in fitness and wellness. She has worked as a personal trainer and fitness instructor for many years. She engages in a regular, well-rounded exercise program. Movement is central to her lifestyle and, in fact, dance was part of her college education. She is currently studying dance movement therapy and clinical mental health counseling on the graduate level. Lisa has two children and lives on the North Shore.
