Primary Care
“I can't afford talk therapy on top of all the other doctor bills, even though it's been recommended by my psychiatrist and primary care doctor. I also don't particularly want to medicate myself to deal with the depression and anxiety. I'd rather learn coping strategies.
Family Practice
I think that's a really good idea, especially if you're ready to start going off the medication. You need guidance. I don't know if a primary care doctor really is trained. Most primary care doctors, whether it is internal medicine or family practice, maybe get six weeks of psychiatry training in their entire residency as opposed to we get 4 to 6 years. We call it know a little bit about everything. Let's say you're run at the mill, anxious or depressed and your doctor started an SSRI and I had a presence. That's fine. But when do you go off it?

I'm going to share a couple stories about how powerful your brain is when it comes to physical symptoms. And of course, I always say you always want to rule out if there's actually a medical problem. A lot of people with panic attacks end up at the emergency rooms. They think they're actually having a heart attack. So if you're feeling like you're having a heart attack, please go get checked out. Just don't assume it's panic if it's really that horrible. But if somebody tells you it's panic, believe them, because I've just had too many clients that have gone to the emergency room, literally four or five times. The doctor keeps saying it's panic, you need to go get therapy, and they don't show up. They don't show up. They just keep going back to the doctor because they just can't believe, which I understand, they just can't believe that this could all be psychological because it's just so powerful. I'm going to share my story. My story goes back to I'm trying to think 1983. I moved out to California from Buffalo, New York. I graduated high school. I moved out here in 1982. I came out here because I have family here.

I hope I'd be like, You know what? I didn't really know that. That makes sense to me now. I'm going to perceive this in a different way just from that little bit of knowledge. We need to take time to be curious with each other, ask questions, understand, Why did you say that to me?, or Why did you do that? Because I was uncomfortable with it. And maybe they need to make an amendment, or maybe it's because what they thought, what was best for them. Or maybe sometimes they think that's what I wanted. And I'm like, No, that's not what I wanted. But that was their perception of it. And I think we've seen this a lot in the last few years, how everybody perceived COVID. Some people were very scared of it. Some people were like, Okay, what do we need to do to handle it? Then a lot of people perceived how some people felt they were forced to take the vaccination. So people's perceptions were like, Thank God I have it. Everybody should take it. So I'm saying these are just some examples I'm giving you that your perception of situations determine your behavior.

And I'm glad she's on the mend. And again, I hope that you can all find some podcasts that touch you or give you some tools that maybe you've been looking for can shift how you're thinking about yourself, others, or the world. I mean, that's what CBT does for all of us. So I'm really grateful for that. So today I wanted to give you a little bit of update on me and kind of share some news I have regarding my recovery with my ovarian cancer diagnosis.
Colon Cancer
Welcome and thanks for being with me. As always, I want to share a review I got this time on Apple podcast. I appreciate you guys that listen off Apple podcast to take the time to put in a review and get the word out. As I always say, my intention here is to get CBT out to the world and help as many people as possible with the tools. And you guys helped me doing that by writing reviews and sharing my posts on Facebook or Instagram. I have a lot of you guys share on your own page, which is super cool because then you can reach all your people. I always wanted to share this one. It says, Thank you, Dr. Osborne. I listened to a couple of newer episodes and enjoyed them so much. I scrolled to the older episodes. I was so grateful to see your series on cancer. I was diagnosed in July 2022 with stage 3 colon cancer.
Prostate Cancer
And we don't want to make assumptions that we know because we don't. But this is really black and white information in concrete and go find out. I know my dad had prostate cancer, but he never had genetic testing. I don't know if it said it wasn't done back then. I don't really know when genetic testing started.

Autism
But it's a tricky diagnosis. Right. So PTSD can be confused with this. Even severe anxiety. People who have characteristics of borderline personality disorder, even people who are on the autism spectrum, can be misdiagnosed as having bipolar disorder.

Stress Management
You want to laugh together, be careful what you're watching. I joke about it now. Cancer is everywhere. And I remember watching a movie that looked very light hearted and the main character ended up having the same cancer as me. And I'm just like, oh my gosh, you've got to be kidding me. It's hard to get away from. So be mindful, try to watch some things, read about them before you put them on. But watching a little movie or TV show that just makes you laugh and allows both of you guys to forget what's going on. You want to do what you can for yourself, and you want to admit to yourself and be honest with yourself what you can't do, right? And it's all good. Whatever you're doing is enough. You want to also use some stress management techniques. Learning how to do some relaxation. Breathing is always good. Just stretching your body is good. Maybe listening to music, relaxation, some guided meditation. So whatever things you can find for yourself to relieve your stress, incorporate those into your time when you have your breaks. Or maybe the person that you're taking care of is having a nap.

The brain is a very powerful organ, and it can affect us in unexpected ways. This can include physical symptoms stemming from mental health challenges.
Psychiatry
Sure. First of all, psychiatry is a medical specialty. Psychiatrists are doctors of medicine. They go to medical school for four years, and then a psychiatry residency is four years, and then an additional two years if you specialize in child psychiatry, like I did. My main practice originally was University of California, Irvine, where I was the program director for child and adolescent psychiatry. Then I left there about 10 years ago, and now I'm the chief psychiatrist at California Institution for Women. A lot of the women that we work with do have childhood issues, a lot of trauma, a lot of anxiety. The child training comes in very well. Psychiatrists mainly diagnose psychiatric disorders and treat patients with medications. We are trained in therapy, but we tend to be rather expensive and very busy because it's an underserved specialty, meaning there's not enough of us. So we generally focus on medication management, and the best psychiatrists work very closely with therapists to help their patients.

And hopefully it might be helpful to you and give you some different ideas on how to think about yourself in the world. So let me first say I'm tapping this in like the middle of January. So as of now, it's been two months that I finished my chemotherapy. I'm really feeling great. I'm just really got good energy.

Pinched Nerve
See me in that place because normally I'm a really strong person. I'm pretty busy. I take care of things. I'm very focus oriented. And I couldn't do anything, especially after my surgery. I mean, I was really helpless. I couldn't really do anything. It was difficult for me to walk because I had a pinched nerve for my surgery and just many issues. So first you have to kind of take that in and seeing the person you care about being sick and seeing what they're going through. And that's why it's really important for you as a caregiver to be able to have people you can talk to, because it's not that this is something you're going to share with the person who's going through treatment, saying you can share with them. It's difficult to see you like this, but you really want to have that conversation with someone else and give you support because the person going through treatment is going through so much as well. And you don't want them to take that in and feel bad that you're helping them. Right. I mean, again, these are all these hotspots people are going to be having when they're going through treatment and also as a caregiver.
Hysterectomy
So because you have a higher risk of serving cancers, you want to make sure you can do the treatments that you may need in the future. Hopefully, that's not an issue I'll ever have to deal with. But just in case, I'm like, oh, that makes sense. That's why they're talking about the future. My other thing I thought of was like, wow, if I had this information 510 years ago and I realized that there was such a higher risk for female cancers, I always just said, I'll have hysterectomy.

Skin Cancer
So that night, Susie went home and she couldn't fall asleep. Being hyper focused on the mold that her doctor noticed, she had catastrophic thoughts that she was probably going to have melanoma and die. She can't wait six months. She needs to go see somebody else. What if you missed something?
Ulcer
And that's when people start to think, right? Or they ate something a little spicy and their stomach's upset. And instead of taking that into consideration, I think now I have stomach cancer or I have an ulcer or what might be happening. I have to go get it checked out and not just kind of going through their day realizing that they're going to have different feelings and things happen to your body, and it's all within normal range. So I once heard someone say that the body is a very noisy place, which I thought I wanted to repeat that and share that with you because I thought the body is a very noisy place.
