Hearing loss (may be asymmetric) with or without dizziness, aural fullness or tinnitus. Symptoms often progress over weeks to months although sudden hearing loss has been reported.67 Occasionally, the patient may have a systemic immune disease such as rheumatoid arthritis.64,65,66 Hughes65 examined 52 patients suspected of having autoimmune inner ear disease and found 7 to have Cogan’s syndrome, 4 with rheumatoid arthritis and 1 having systemic lupus erythematosus.65 Cogan’s syndrome is a rare disease characterized by non-syphilitic interstitial keratitis associated with tinnitus, vertigo and hearing loss.68 The etiology is uncertain but evidence suggests that immunologic or infectious causes are likely.69

Binaifer is certified in Vestibular Rehabilitation (1998), is a CranioSacral Therapy practitioner since 1996 and currently holds a Diplomate (advanced) level certification in CranioSacral Therapy (CST) from the Upledger Institute. Primary Care
Referrals are obtained from your Primary Care Physician. Please give them 10 days notice of your need for a referral except when it is an emergency. The paperwork may be faxed to our office for the visit. Verify prior to your visit if you have a question about our receiving the referral.
Ear Infections
Pediatric Otology is focused on the diagnosis and treatment of ear-related problems in children. These problems can include hearing loss, ear infections, congenital anomalies of the ear, and balance disorders.
Emergency Care
For our patient’s needs, we do take Nurse Calls and prescription refills on a daily basis. These calls will be answered as quickly as possible and will be screened for emergency. True emergency calls should be made to 911 or you should go to the nearest Emergency Room for service.

Central dizziness is caused by problems in the balance portion of the brain. Anytime this portion of the brain is not working properly, dizziness can occur. Symptoms usually include lightheadedness, disorientation, imbalance, and sometimes even blacking out. Causes of central dizziness include low blood sugar, low blood pressure to the brain, strokes, multiple sclerosis, migraine headaches, head injury, tumors, and the aging process, among others. Treating these types of dizziness usual involves treating the problem which is causing the brain to not work properly.

Hypothyroidism
The most frequent laboratory abnormality associated with vestibular dysfunction in hyperglycemia. Frequently dietary manipulation will lead to a beneficial response.118,119 Spencer120 reported on 1,400 patients with hearing loss and vertigo who were diagnosed with hyperlipidemia. Hypothyroidism has also been implied as a potential cause of neuro-otologic dysfunction.118

Ultrasound
Dizziness can be caused by numerous disturbances to any of many different parts of the body. Based upon your history and physical findings your physician may require further tests to complete a full evaluation. The tests necessary are determined at the time of examination and may include hearing and balance tests, imaging (CT or MRI scans), blood tests, and ultrasound tests. A general physical exam and neurological tests may also be needed.
MRI
Tumors of the hearing and balance nerves arising between the inner ear and brainstem have been extensively studied at the Michigan Ear Institute. Modern diagnostic testing including auditory brainstem evoked response testing and MRI testing are currently in use. Surgical removal of these tumors has resulted in excellent quality of life with minimal side effects. “I went to Dr. Eleanor Chan, for an ongoing ear infection in 2016. I ended up having surgery to remove eroded tissue and repair my ear canal. She was very caring, gentle and gave detail information so I could understand. She did an exceptional job on the surgery. I felt no pain following the surgery, unless I laid on that side of my head. It was a pretty quick recovery, also. Currently, Oct, 2020, I need surgery again because I have some damage to the tissue inside the same ear. I may have possibly caused by using q-tips. Not really sure but I will be getting a ct scan,but, once again, Dr. Chan, was caring, informative. She is very professional, kind and confident and I trust her 100%. Even the entire staff and anyone I have spoken to on the phone, are wonderful to deal with.” K.A.

General Surgery
Dr. Bojrab received his Medical Degree from the University of Indiana, School of Medicine, Indianapolis, Indiana. His residency was completed at the University of Indiana. He performed his general surgery training at Butterworth Hospital, Grand Rapids, Michigan, prior to completing his Otolaryngology training at Indiana University at Indianapolis. His training in Otology, Neurotology, and Skull Base Surgery was at the prestigious Otology Group, Nashville, Tennessee associated with Vanderbilt University, under the direction of Michael Glasscock, MD.
Ear Surgery
The fellow will participate in caring for all otologic surgical patients in the clinic, inpatient, outpatient, and operative settings. The fellow’s hands-on operative experience will be focused toward all surgical procedures performed by the staff physicians including assisting in chronic ear surgery and stapes procedures. The majority of the skull base cases consist of acoustic neuroma operations, cochlear implants, vestibular nerve sections and labyrinthectomy, and an assortment of complex facial nerve and lateral skull base surgery procedures. Translabyrinthine, retrosigmoid, and middle cranial fossa approaches are used. Newer advances are also utilized, such as stereotactic radiotherapy and endoscopic approaches.
Neurosurgery
Infection-Infections are a rare occurrence following facial nerve surgery. Should an infection develop after a middle fossa it could lead to meningitis, an infection in the fluid surrounding the brain. Fortunately, this complication is very rare.
Otolaryngology
The Michigan Ear Institute Fellowship in Neurotology/Skull Base surgery has produced some of the finest neurotologic surgeons in the country. A combination of the excellence of the program of study, the caseload available, and the subsequent success of our high quality graduates has made the reputation of the fellowship exceptional. Our fellow graduates hold leadership positions in both private practice and academic practice groups as well as many national organizations including the American Otological Society, the American Neurotological Society, and the American Academy of Otolaryngology Head and Neck Surgery, among others. They also serve as chairman of several otology programs across the country.

Plastic Surgeons

Botox
Surgery to correct this problem may involve 1) intentional weakening of the nerve through an incision on the face which may also require a gold weight eyelid implant, or 2) relieving pressure on the nerve adjacent to the brain. This operation includes risks of facial paralysis, hearing loss, dizziness, spinal fluid leak and stroke. 3) An injection of Botox can give temporary relief. As always, you should discuss your proposed surgery in detail with your doctor.
Eyelid Surgery
Implantation of gold weights in the upper eyelid can be helpful in counter balancing the lifting eyelid muscle. This helps to prevent dryness and irritation of the eye and helps to improve the appearance. Shortening of the lower lid or corners of the eyelid are sometimes performed at the same operation. Complications of these procedures are rare and may include bleeding, infection, droopy eyelid, extrusion of the implant and possible visual loss.
Face Lift
There are a variety of other operations that are sometimes performed by themselves or in combination with the above procedures. Such procedures may include a face lift or removal of excess skin at the brow or cheek. For patients who have some faulty return of facial function, selective cutting of facial nerve branches or facial muscles may be of benefit and may be combined with other procedures to correct some of the anticipated weakness.

Ulcer
The most serious complication that may develop as the result of total facial nerve paralysis is an ulcer of the cornea of the eye. It is important that the eye on the involved side be protected from this complication by keeping the eye moist. Closing the eye with the finger is an effective way of keeping the eye moist. One should use the back of the finger rather that the tip in doing this to insure that the eye is not injured.
Eczema
We order these tests on patients who can not or do no want to undergo skin testing. Patients with uncontrolled asthma, severe eczema or hives are best evaluated with RAST to avoid potentially serious side effects.

Dr. Babu is a board certified in Otology, Neurotology, Otolaryngology and Skull Base Surgery. He attended the accelerated program for combined B.S./M.D. studies at Northeastern Ohio Universities College of Medicine. Dr. Babu completed his Otolaryngology training at Henry Ford Health System in Detroit, MI and his fellowship at the Michigan Ear Institute. He has clinical interests in ear surgery, hearing loss and reconstruction, dizziness, cochlear implants, acoustic neuromas, skull base lesions and facial nerve weakness in children and adults. Dr Babu has numerous articles and book chapters written in these areas.
