They are located within CCH at 701 S. Holly St. in Collins.Seniors often experience significant changes in their lives. As these changes take place, a behavioral health program can help maintain one's quality of life. Our Senior Care Unit enables adults 65 or older to improve their daily functioning. Referrals can be made directly to the unit. A Senior Care professional is available to provide a confidential assessment in the home or at our facility.We are pleased to announce that the Family Care Main Street in Collins, and Family Care Clinic of Seminary are now open for business. We are grateful to be part of these towns and are excited to bring another benefit to these two communities. Dr. Ginger Pace-Herndon, formerly of Wesly Medical Center, is your physician at the Seminary clinic. Jamey Shivers FNP, of CCH, is now seeing patients at the Main Street clinic.

        i went in through the emergency room. beware their doctors are not part of the hospital and no one will tell you. the hospital knows they are not part of their network. they say it is not their responsibilty but keystone says the hospital is suppose to tell you. you get a huge doctor bill because of it. the hospital assistant admin. told me that keystone said that they will "greatly reduce my bill" but keystone says that was never said.
