Golden Horizons Elder Care Services, Inc., founded in 1996 by Lisa Gradzewicz, is an in-home care agency that provides quality care to the elder population along the Connecticut Shoreline and into the Greater Hartford area. We serve four counties: Hartford County, Middlesex County, New Haven County, New London County. Mission - Our mission is to help seniors live an independent lifestyle while receiving the quality care they need. Caregivers - At Golden Horizons, we only hire thoroughly screened and experienced caregivers. All caregivers are bonded and insured with professional liability and worker's compensation, and are employees of Golden Horizons. Prior to hiring, every potential caregiver undergoes thorough national criminal background check, driving record check with the Department of Motor Vehicles, drug test with a zero-tolerance policy, personal and business reference check, and an extensive interview. Once hired, each caregiver is required to have in-service training annually. Facts Golden Horizons opened in 1996. Our staff is comprised of CNAs, HHAs, homemakers, and companions. We use a phone monitoring (telephony) system to account for all hours worked. We are registered with the State of Connecticut as a Homemaking/Companion agency. State of CT Reg. #HCA.0000133. Fully bonded and insured. We do work with assisted living, residential living, independent living, rehabilitation and healthcare centers, and private residences. We are BBB accredited since June 2010, with an A+ rating. We provide 24/7 care. We do not accept Medicaid or Medicare. Services/Products

Golden Horizons Elder Care Services, Inc., offers personal care, homemaking, and companionship services on an hourly or live-in basis. 24/7 care is available. Prior to the start of all services, we visit the client and conduct a complete assessment of their individual needs. We then work with the client, family members, and medical professionals (when appropriate) to create a customized, comprehensive plan of care to meet the client's needs. Adjustments can be made to the plan at any time going forward. Homemaker/Companion: Homemakers provide a range of services including household chores, meal preparation, shopping, and companionship. Homemakers/Companions do not provide personal care such as bathing and dressing. Duties include: Light housekeeping Vaccuum/dusting Laundry Change bed linens Meal preparation Dishes Kitchen & bathroom maintenance Transportation - appointments, errands, Senior Center etc. Companionship - walks, games, conversation etc. Personal Care Attendant: Personal Care Attendants provides personal care such as bathing and dressing, and all the duties of a homemaker/companion. Duties include: Assistance with bathing Assistance with toileting Assistance with hair care Assistance with foot care Assistance with skin care Assistance with medication reminders Hospice Care Assistance: We assign only our most experienced and highly qualified caregivers to these most difficult situitions. Live-In Companion & Personal Care Attendant: We provide live-in services. Live-ins provide the same services as hourly caregivers. A 3-day minimum is required for live-in services. The client is expected to provide suitable living quarters and meals for the live-in. Peapod Grocery Shopping Online: We offer online shopping, through Stop & Shop's Peapod Online Shopping, for your convenience. Medication Reminders: We offer assistance with remembering medications.
