Non-invasive methods include wearing compression stockings, exercising, leg elevation, losing weight, not wearing tight clothes, avoiding long periods of standing or sitting, and not crossing your legs while seated. ESES (pronounced SS) is an easy way to remember this conservative approach. It stands for Exercise Stockings Elevation and Still. Exercise, compression hose/stockings, elevation and rest will not make the veins go away or necessarily prevent them from worsening due to the underlying venous reflux disease not being addressed; however, it may provide temporary relief of the symptoms associated with varicose veins. Weight reduction is also helpful. If there are inflamed areas or an infection, topical and/or oral antibiotics may be prescribed.

Leg swelling is a special area of interest of Dr. Polly Kokinos, and she has been active in doing clinical research to find better ways to diagnose and to treat this condition. During her residency, Dr Kokinos returned to New York to train in molecular carcinogenesis research and published several articles on breast and thyroid cancer pathogenesis. During her last year in San Francisco, a nurse from the hospital introduced her to a high school friend at a Greek Church Festival; Polly and Michael Kokinos married the following year and are proud to have been married for 28 years.

Ultrasound
All the studies we perform at the Institute for Vascular Testing are pain-free, non-invasive, and performed by a Registered Vascular Technologist (RVT) who is specially trained to conduct ultrasound studies of blood vessels. We use a state-of-the-art Phillips Envisor Ultrasound machine to take pictures of blood vessels and blood flow, utilizing a small probe with some warm gel on the area of interest. There are no needles or radiation. Most studies take under an hour.

Aneurysms are usually asymptomatic but can be diagnosed as a pulsatile (beating) mass in the abdomen, by chance on a CT scan or x-ray, or on a screening ultrasound. When they rupture, they can cause severe abdominal, back, or flank pain, dizziness, rapid heartbeat, or shock.

General Surgery
After graduating from high school at 14 years old, Dr. Kokinos enrolled at Barnard College (Columbia University), finished in 3 years, and earned a coveted spot in Columbia University’s prestigious medical school when she was 17 years old. She finished medical school at 21 and was accepted at UCSF Surgical, the top ranked general surgery residency program in the world.
Wound Care
Leg and foot wounds that don’t heal normally may be a sign of an underlying circulation issue. The problem may be either arterial (delivering oxygen and nutrients to the affected area) or venous (removing toxins and fluids). Unfortunately, many doctors and hospital wound care centers may not properly diagnose the problem, which may increase the risk for amputation.
Minimally Invasive Surgery
Many aortic aneurysms can now be repaired by inserting an endovascular stent through the groin. The stent is actually made up of three sections that are inserted and then connected to “bypass” the aneurysm and keep blood flowing from the main aortic artery to the right and left iliac arteries. This minimally invasive surgery is done in a hospital but generally requires only an overnight stay, with recovery taking several weeks. Dr. Kokinos is experienced and highly skilled at this delicate procedure.
Gangrene
Hand ischemia: The fistula as it grows, or graft when it is placed, diverts some of the blood that would normally go to the hand to the access. In some patients, particularly those with diabetes, this can result in not enough blood going to the hand. This can cause fingers that are numb, cold, and painful, and may (rarely) develop gangrene. It is important to have this evaluated urgently by your surgeon if it happens.

Ulcer
Venous Wounds: Venous ulcers are the most common type and are located in the middle or lower part of the leg, usually above the ankle. These ulcers may be quite large (5-10 cm diameter) and may be “ugly” in appearance, but they are usually not painful. If there is associated pain, the pain is frequently relieved by elevation of the extremity. The skin around these ulcers may be brownish in color and may be firm to the touch (not soft and supple) for many years before the ulcer begins. These types of problems can be treated permanently, and risk of limb loss is low.
