Primary Care
Dr. Svetlana Spivak has been a Pediatrician for over 30 years. She graduated from the Leningrad Medical School of Pediatrics and completed her post-graduate training at a prominent Infectious Disease Hospital. She finished the Mount Sinai School of Medicine Residency program at Elmhurst Hospital and has been a Primary Care Pediatrician ever since.
Family Practice
Patient Forms Section. They include: Patient Registration, Family Health History, and an Authorization for Information Release. Please fill out the Authorization for Information Release only if you are coming to us from another doctor. This form allows us to obtain medical records from the patient’s previous medical provider.
