Upon completion of a Fellowship in Reproductive Endocrinology, a specialist seeks Board certification, a multi-step process. To become Board certified in Reproductive Endocrinology, the physician must first obtain Board certification in Obstetrics and Gynecology. This requires successful completion of both a written and an oral examination. Board certification in Reproductive Endocrinology requires successful completion of additional written and oral examinations. The entire certification process takes several years to complete. Only a physician who has successfully completed a Fellowship in Reproductive Endocrinology and passed the examinations can become Board certified as an infertility specialist.
Endometriosis
Almost 90% of couples conceive after a year of unprotected intercourse. Evaluation of infertility is indicated if the female partner is under 35 years of age and the couple has been trying to conceive for one year. Immediate evaluation and treatment of infertility is appropriate in cases of known problems that impact fertility such as irregular menstrual cycles, pelvic endometriosis, past history of pelvic infection or sexually transmitted disease, previous abdominal or pelvic surgery, or severe male factor infertility.

When endometriosis infiltrates into the upper vaginal wall, rectal wall or other tissue in the deepest part of the pelvic cavity, it irritates the nerve endings in the region, causing pain with sex, bowel movement or chronic pelvic pain.

We have the distinct privilege of having an on-site laboratory director who is double boarded as a reproductive endocrinologist and a high complexity laboratory director. She is uniquely qualified with the technical skills in embryology and andrology, the laboratory science in human andrology and embryology, and the medical expertise in reproductive endocrinology and infertility. Our laboratory director is involved not only in the day-to-day operation but the critical quality improvement activities in the laboratory. Her unusual qualification and dedication enables us to critically evaluate every new finding for its merit before we integrate seamlessly a new clinical discovery or ground-breaking innovation in assisted reproductive technologies into our practice and our laboratory.

Ultrasound
During this first appointment, our physician will take a detailed medical history, review all your past infertility testing and treatment, and where necessary perform a pelvic ultrasound.
MRI
MRI does not use ionizing radiation like X-rays or computed tomography (CT). An MRI scanner uses a combination of electromagnetic field and radio frequency to allow a construction of an image of the scanned area of the body. On some occasions when rectal or vaginal nodule is found, an MRI of the pelvis may be performed to help differentiate deeply infiltrating pelvic endometriosis from other diseases. Like ultrasound, MRI may be helpful to tell the difference between an ovarian endometrioma from other ovarian masses, but has limited value to diagnose superficial endometriosis growth.

Hysterectomy
Hysterectomy is the removal of the uterus. The ovaries may or may not be removed. Endometriosis is less likely to come back if the ovaries also are removed. Hysterectomy is a last resort when medical treatment and conservative surgery have not worked, extensive disease, and a woman no longer wishes to have any children. Unfortunately, in spite of hysterectomy and removal of the ovaries, there is a small chance that pain symptoms will come back.
Laparoscopy
Both tests can uncover uterine abnormalities such as adhesions or polyps inside the uterus, or fibroid that protrudes or distort the uterine cavity. But, only the HSG can evaluate tubal abnormalities such as blockage or occlusion. A hydrosalpinge is a blocked fallopian tube distended by fluid within. Abnormalities on an HSG or sonohysterogram may warrant further evaluation with laparoscopy and or hysteroscopy.

Currently there is no cure for PCOS. Symptoms of PCOS can be managed with medications, dietary modification and a healthy lifestyle. Our patients see our reproductive endocrinologist, Dr. H. Christina Lee, who often consults with other physicians and experts in dermatology, nutritional science, psychology and genetics. We work together with each patient to create an individualized treatment plan that works for her.

Endometriosis responds to changes in hormones. It breaks down and bleeds like the lining in the uterus (endometrium) during the menstrual cycle. The localized bleeding of endometriosis lesions in the pelvis, combined with the inflammatory chemicals released by specialized white blood cells in the pelvis lead to pelvic pain.
