Medical Weight Loss
While the diagnosis of PCOS is based on clinical assessment not laboratory tests, a number of different hormonal irregularities occur in subsets of women with PCOS. Very few women manifest all of these laboratory abnormalities which can include higher levels of LH than FSH, more male hormones (testosterone) relative to female hormones (estradiol and, especially, progesterone) and elevated fasting glucose and/or insulin. Women with more severe PCOS are at an increased lifetime risk of developing diabetes, hypertension and heart disease. Young PCOS patients who are overweight may achieve remarkable success with even modest weight loss.

Male Infertility

Infectious Diseases
The live birth rate per cycle is about 10-12%. The likelihood of success with IUI depends on the type and severity of the infertility problem and the woman’s age. According to California Health and Safety Code Section 1644.5, male partners must be checked for the following infectious diseases before the IUI procedure can be performed: HIV (causative agent of AIDS), hepatitis B & C, HTLV and syphilis (RPR). Sperm donors, whether directed or anonymous, undergo a much more rigorous screening and testing process dictated by the FDA regulations.
Electrocardiogram
Since many of our recipients are beyond the usual childbearing age, a medical and obstetrical pre-conception evaluation is advisable to assess the safety of initiating an IVF pregnancy. A mammogram is recommended for all women above the age of 40. Above the age of 48 years a stress/exercise EKG or echocardiogram is done.

Gynecology
Dr. Richard Chetkowski founded the Alta Bates IVF Program in 1984. He received his MD degree from UCSF, then completed a residency in obstetrics and gynecology at Yale and a fellowship in reproductive endocrinology and infertility at UCLA.
Female Infertility
There is strong evidence linking smoking with both male infertility and female infertility, increased chance of miscarriage, retarded fetal growth, placental abruption and other unfavorable pregnancy outcomes in addition to the well-known general health hazards. We cannot recommend strongly enough that both partners quit smoking because passive smoking carries many of the same risks. Once pregnancy is established, alcohol is contraindicated and caffeine intake by the mother needs to be kept below 200 mg per day (the content of one cup of coffee or one cola soft drink).

Labor and Delivery
The most important complications associated with multi-fetal pregnancy are preterm labor and delivery, pre-eclampsia and gestational diabetes. Prematurity accounts for most of the risks associated with multiple gestations. Poor fetal growth and discordant growth among the fetuses also result in perinatal morbidity and mortality. Obstetrical complications, such as placenta previa, placenta abruption and postpartum hemorrhage are more common in multi-fetal pregnancies.

Some patients strongly prefer to conceive with eggs donated by a close relative who is their directed/known egg donor. While such donors are often older than ideal, they need to have reasonable ovarian reserve and pass the FDA screening. In-family egg donation can be a highly satisfactory solution but it can have lasting effects on the family dynamics. Therefore, assessment and counseling by an experienced mental health professional is required for optimal outcome.

Ultrasound
I. Ovarian Factors: AMH (Anti-Mullerian Hormone), Cycle day 3 FSH and Estradiol, Cycle day 21 progesterone. Ultrasound assessment of antral follicle count (AFC).

Chemotherapy
In some cases the disease itself may impair future fertility while in others it is the treatment, such as chemotherapy, which may impact the ability to have children in the future. While each patient and each condition requires a highly individualized approach, a few general rules apply to many situations.

Vasectomy
If the ejaculate lacks sperm, the condition may be due to either obstruction, such as following a vasectomy procedure, or failure of sperm production in the testes. The former condition is known as obstructive azoospermia and it is the primary indication for MESA procedure.

In some instances the first HCG test is higher than the second one which indicates a “biochemical” pregnancy which ceases to evolve at an early stage. If your pregnancy progresses normally, you will be scheduled for an ultrasound examination about 4 weeks after retrieval. About 2-4% of pregnancies are ectopic, i.e. in the tube.
