Johnson County Community Health Services (JCCHS) has been improving your quality of life since it was established in 1975. JCCHS was created for the multiple purposes of health guidance, prevention and control of disease in the homes and communities of Johnson County, Missouri. JCCHS operates under County Health Center Statutes, Chapter 205, RSMo. Under its elected Board of Trustees, JCCHS serves as the County Health Center and operates three divisions: MCH accredited Public Health; a Home Health care agency and Johnson County Hospice Care; both federally certified and state licensed. Our Home Health and Hospice divisions additionally provide care to Lafayette county residents in their homes. JCCHS strives to improve the quality of life of our clients, their families and the community by providing our services under the supervision of licensed professional nurses and physician orders. Our Home Health team promotes independence and recovery in the home setting and our Hospice care division utilizes a multi-disciplinary team of professionals to provide quality end of life care. All JCCHS divisions work collaboratively with area community groups and providers to address a wide range of health care needs to eliminate health disparities and increase the number of healthy years that Johnson county residents live.
