Biologics are protein-based medications that interact with the immune system to reduce the inflammation causing psoriasis. Newer biologics are highly targeted to affect only certain parts of the immune system. The first group of biologics developed were the tumor necrosis factor-alpha (TNF-alpha) mediators. Since then, interleukin 17-A, 12, and 23 mediators have been also developed. These biologics are approved for psoriasis, psoriatic arthritis, or both.
Cryotherapy
Cryotherapy is the use of liquid nitrogen to destroy skin cancer cells by freezing the cells to extremely low temperatures. This freezing disrupts the superficial layers of the skin and causes these skin cells to separate and fall off. Careful placement of the liquid nitrogen specifically on the skin cancer cells is important to avoid damaging surrounding tissue. This technique is very useful on superficial skin cancers and pre-cancers. Deeper tumors may not respond to cryotherapy due to the inability of the cold to penetrate too deeply without damaging surrounding tissue.

Hyperpigmentation is caused by an overproduction of pigment called melanin. The two main types of hyperpigmentation are post-inflammatory hyperpigmentation and melasma. Pot-inflammatory hyperpigmentation typically occurs after an injury to the skin and the skin reacts by making extra pigment. The type of injury could be from physical trauma, such as a cut, rubbing, or a burn. Or the injury can be from an inflammatory rash or irritant. Melasma on the other hand is typically hormonal and can occur after childbirth or with oral contraceptives. But, melasma can be worsened by skin injury or sun exposure.

Topical chemotherapy involves the use of creams to treat skin precancers or cancers. These medications include medications like 5-fluorouracil, imiquimod, picato, and solaraze. In general, these medications are effective in treating precancers or superficial skin cancers, but do not penetrate deep enough to treat invasive skin cancers.

Textural unevenness can be treated with fractional laser resurfacing or Renuvion resurfacing.

Phototherapy is the use of specific wavelengths of light that have been shown to be very effective in the treatment of psoriasis. This light occurs naturally as a component of sunlight and is called ultraviolet light. It is also very effective in the treatment of many other skin conditions, such as pruritis (itching), atopic dermatitis (eczema), contact dermatitis, alopecia (hair loss), lichen planus, and dyshidrotic eczema.
Fungus
ADSCI has recently acquired the Cooltouch laser, which is FDA-approved for use on the skin. It has recently has been used for the treatment of toe nail fungus. The principle behind the treatment is the heating of the nail bed and nail plate to about 40C. This kills the fungus and the spores. We recommend three Cooltouch laser treatments once a month apart combined with a topical anti-fungal lacquer. We have seen fantastic results with this new anti-fungal treatment. The advantage of this treatment is that oral anti-fungals, which could have potential side-effects, are not needed.

Microdermabrasion
The foundation for a long-term treatment plan for hyperpigmentation is a good skin care regimen. A vitamin C cleanser and moisturizing sunscreen with broad-spectrum SPF coverage of 30 or higher is beneficial. A hydroquinone based bleaching cream to lighten the skin is the most effective method to rapidly lighten the dark spots. There is some controversy requiring the safety of using high dose hydroquinone, so we try to minimize the use. Adding chemical peels, microdermabrasion, intense pulsed light, and lasers can help improve the hyperpigmentation faster.
