Founded in 1919, the Stratford Visiting Nurse Association, formerly known as the Stratford Public Health Nursing Association, is one of the largest, independent, nonprofit organizations that serves the needs of families throughout Fairfield and New Haven counties in Connecticut. The association accepts insurances from Aetna, Anthem Blue Cross and Blue Shield and Medicaid. It is certified by Medicare, licensed by the state s department of health services, as well as accredited by The Joint Commission. The Stratford Visiting Nurse Association provides a range of health services for individuals confined to their homes. It offers the Honeywell-HomMed Telemonitoring Program and End of Life Care program.
