        Dr. Greg Walters DDS - Siloam Springs Arkansas. This office flubbed the insurance claim on services provided to my wife. They did not notify us of non-payment until 8 months later, threatening us with collection. Had to pay them cash to prevent a negative on our otherwise flawless credit report.
