Obesity, which is present in approximately 30 to 50 percent of patients with PCOS, is an independent cause of insulin resistance that may result in a more severe degree of insulin resistance in women with PCOS who are also overweight. Unfortunately, endometriosis is not visible on x-ray and cannot be seen with ultrasound unless a chocolate cyst is present within the ovary. For this reason, the diagnosis of endometriosis must be confirmed by laparoscopy, an outpatient surgical procedure, performed under general anesthesia, during which a telescope-like instrument is inserted through a small incision in the belly button to visualize the implants of endometriosis and any adhesions that may be present within the pelvis. In most cases, removal of endometriosis implants, endometriomas and scar tissue can also be performed during the same procedure. More advanced cases, which may not be technically feasible or safe to correct with laparoscopy, may require more extensive surgery through a larger incision. Other tests that may be helpful in selected cases include CT or MRI imaging of the pelvis, intravenous pyelography (IVP) and cystoscopy to examine the ureter and bladder, and barium enema or colonoscopy to evaluate the intestinal tract.

Endometriosis
Endometriosis refers to the condition in which cells from the endometrium, the tissue that normally forms the lining of the uterine cavity, become embedded in areas of the body outside of the uterus. When present, small implants of endometriosis, measuring several millimeters in diameter, are most commonly found in the lower abdomen of women, either on the inside lining of the pelvis or on the surface of the ovary, uterus or intestine.

The requirements to perform IVF are less strict for two reasons. First, the IVF procedure does not require the use of the fallopian tubes and is commonly used to bypass problems with tubal damage, absent tubes or pelvic scar tissue that results from infection or endometriosis. There is one concern, however, about fallopian tubes that are blocked at the far end, near the ovary, from previous infection. There is evidence to suggest that in the presence of this blockage, tubal fluid may drain back into the uterus and decrease the chance that the embryos placed in the uterine cavity during the IVF process will implant normally, reducing pregnancy rates. Opening these damaged tubes surgically, while a possible option, may not be the best choice due to the increased risk of tubal pregnancy that can result. When the tubes are thought to be damaged beyond repair, it is generally recommended to perform a laparoscopy and either remove the tube or create a second blockage at the near end of the tube, next to the uterus, to prevent the fluid from entering the uterine cavity.

Following a one-year internship in Internal Medicine and General Surgery, he began a three-year residency in Obstetrics & Gynecology, all at the Brown University Program in Medicine. In 1983, Dr. Littman completed a two-year fellowship in Reproductive Endocrinology at the National Institutes of Health. From 1983 until 1987, he was on the full-time teaching faculty of the George Washington University School of Medicine. Since 1987, he has been in private practice in Rockville, MD, while continuing as a Clinical Professor of Obstetrics & Gynecology at George Washington University.

The current criteria for the diagnosis of PCOS require the presence of two of the following three characteristics: (1) infrequent menstrual periods; (2) increased androgen production, usually signified by the presence of acne or increased male-pattern hair growth on the face, chest, back or abdomen; and (3) ovaries that appear polycystic on ultrasound. Some women with PCOS will have normal appearing ovaries on ultrasound. Conversely, polycystic appearing ovaries may be present in women with normal menstrual cycles. There are no laboratory tests that are diagnostic of PCOS. Rather, laboratory testing is used to rule out other causes of ovulation failure, such as elevated prolactin levels, thyroid problems, or other causes of excessive androgen production, such as ovarian or adrenal gland tumors.
MRI
Some women, even with multiple endometriosis implants and extensive scar tissue formation, are remarkably pain-free. In these women, endometriosis may only be found as part of an evaluation for infertility or by the coincidental finding of an endometrioma during an ultrasound, CT scan or MRI of the pelvis that was performed for other reasons.
Radiology
The hysterosalpingogram (HSG) is an x-ray procedure that is performed to ensure that the cavity of the uterus is normal and also to confirm that the fallopian tubes are open. The test is performed in the radiology department of the hospital just after the menstrual bleeding has stopped in order to prevent x-ray exposure during an early pregnancy. Special dye, which is opaque to x-ray, is injected through the opening of the cervix. With the fluoroscope, the dye can be seen to outline the cavity of the uterus and flow into the fallopian tubes. If the tubes are open, the dye will spill freely into the abdominal cavity.

In selected cases when there are no sperm present in the ejaculate, it may be possible to obtain sperm directly from portions of the sperm duct known as the vas deferens and epididymis or from the testes. This is most likely to be successful in cases where sperm production by the testes is normal but the sperm ducts are blocked, either from prior infection or vasectomy. In a procedure known as percutaneous sperm aspiration (PESA), a urologist will attempt to insert a small needle into the duct to withdraw sperm that can be used for an egg retrieval scheduled for later that day or frozen for use at a later time.
Laparoscopy
Birth control pills are commonly used as initial therapy for women suspected of having endometriosis who wish to defer laparoscopy or to prevent recurrence of the disease after surgical treatment. Women who take birth control pills tend to have lighter menstrual periods with less cramping and may be less likely to reseed new areas of endometriosis within the pelvis. Low-dose birth control pills can be used in a cyclic fashion, with a rest period to allow a menstrual period to occur, or can be given continuously to avoid menstruation for women with persistent pain during their periods. Although there is a theoretic concern that the estrogen component of the birth control pill may potentially stimulate growth of the endometriosis, this effect appears to be balanced by the progestin component of the pill, which tends to inhibit the estrogen effect on endometriosis.
