To figure out the cause of foot pain, podiatrists utilize several different methods. This can range from simple visual inspections and sensation tests to X-rays and MRI scans. Prior medical history, family medical history, and any recent physical traumatic events will all be taken into consideration for a proper diagnosis.

Foot and ankle surgery may be performed depending on the severity of the injury. In any case, if you happen to develop a foot condition, it’s always advised you seek the counsel of a podiatrist for a proper diagnosis and advised treatment plan. If you notice an abnormal growth, a change in the appearance of your foot or ankle, discomfort while standing or walking, or general foot pain, it would be in your best interest to seek professional help, especially if surgery is the recommended treatment for your case.
Bunions
Bunions are most commonly formed in people who are already genetically predisposed to them or other kinds of bone displacements. Existing bunions can be worsened by wearing improperly fitting shoes. Trying to cram your feet into high heels or running or walking in a way that causes too much stress on the feet can exacerbate bunion development. High heels not only push the big toe inward, but shift one's body weight and center of gravity towards the edge of the feet and toes, expediting bone displacement.
Ingrown Toenails
Signs that may indicate the formation of an ingrown toenail include redness, pain, swelling of the surrounding skin, possible bleeding or oozing of pus, and an overgrowth of the skin around the nail. The surrounding skin may also feel tender and become swollen or hardened. Most ingrown toenails are diagnosed by a simple physical exam; however, an x-ray may be required to show how deep the nail has grown into the skin of the toe.
Reconstructive Surgery
Ankle sprains are the most common sports related foot and ankle injury. A sprain occurs when the ligament holding the ankle bones and joint stretches beyond its normal range. It can be treated non-surgically with a combination of rest, ice, compression by a bandage, and elevating the ankle. This combination is referred to as RICE. Severe ankle sprains in which the ligaments are torn may require arthroscopic or reconstructive surgery followed by rehabilitation.

Corns
Corns and calluses are hard layers of skin that usually form due to friction. Generally, they will be in areas of the foot that are bearing weight, or areas that frequently rub against the shoe. Soft corns are typically found between the toes.
Ulcer
Those with diabetes are at serious risk due to being unable to feel an ulcer on their feet. Diabetics usually also suffer from poor blood circulation. This can lead to the wound not healing, infections occurring, and the limb may have to be amputated.
Calluses
If you have arthritis, frequently wear tight fitting shoes such as high heels, and are older in age, it’s very likely that you may develop a bunion. A bony hump at the base of the big toe is the biggest sign that you may have developed this condition. Bunions are often painful and typically cause an extreme amount of discomfort. Because a bunion protrudes on the side of the foot, it’s likely for it to continuously rub against the inside of your shoe. This causes frequent friction and pain, which may cause calluses to form as well.
Fungus
Fungal toenails form due to an infection that gets in between either the cracks in the nail or possible cuts on the surrounding skin. When this occurs, it’s likely to discolor and thicken the nail; this makes it hard to care for or trim. The nail itself may also become brittle and could potentially crumble. The fungus under the nail may build over time, separating the nail from the bed and ultimately causing it to fall off if not treated properly.

Podiatrists perform foot and ankle surgery to treat a number of different conditions. These may include bunions, heel pain, flat feet, arthritis of the foot or ankle, sports injuries, hammertoes, ingrown toenails, complications due to diabetes, and fractures, to name a few. Again, the need for surgery is dependent upon the type of injury you are experiencing, as well as its severity. This also means that the recovery process for surgery will vary as well. With that being said, most podiatrists will recommend you get plenty of rest, stay off your feet, ice the affected area to reduce swelling, and elevate the affected area.
Physical Therapy
There are various options your podiatrist may suggest for heel pain. Treatment options for heel pain typically include non-steroidal anti-inflammatory drugs (NSAIDS), which may reduce swelling and pain. Other options are physical therapy, athletic taping, and orthotics. In severe cases of heel pain, surgery may be required.

Achilles Tendonitis
The achilles tendon is the largest tendon in the body. It connects the calf muscles to the heel bone. Running, jumping, and walking can all impact this tendon. Two common injuries to the Achilles tendon are tendonitis and a rupture of the tendon. Achilles tendonitis is inflammation in the tendon often caused by an increase in the amount and intensity of stress placed on it. It can be treated non-surgically with rest, ice, or anti-inflammatory medication. In some cases, surgery may be required. A rupture of the Achilles tendon can be treated by placing the lower leg in a cast for several weeks or with surgery. Both methods require 4 to 6 months of rehabilitation.
Ankle Instability
Sports in which ankle sprains commonly occur include soccer, tennis, and cheerleading. Ankle sprains are caused by a tear in the ligament of the ankle, which typically occurs when the foot is rolled in an awkward manner. Ankle sprains are usually accompanied with bruising, swelling, and pain. Those who have sprained an ankle in the past may run the risk of developing chronic ankle instability. Those with this condition will generally feel a persistent pain, swelling, and tenderness in the ankle, and may experience the sensation of the ankle giving way when performing everyday activities.

Bunions Problems
A podiatrist knowledgeable in foot structure and biomechanics will be able to quickly diagnose bunions. Bunions must be distinguished from gout or arthritic conditions, so blood tests may be necessary. The podiatrist may order a radiological exam to provide an image of the bone structure. If the x-ray demonstrates an enlargement of the joint near the base of the toe and a shifting toward the smaller toes, this is indicative of a bunion.

Flat Feet
Fallen arches is a condition that is termed posterior tibial tendon dysfunction, or PTTD. It also may be referred to as adult-acquired flat feet. Many people with flat feet suffer little or no problems. But others may experience pain if a tendon in the arch of the foot becomes inflamed, overstretched, or torn. The problem starts with the posterior tibial tendon, which begins in the leg, runs through the ankle, and connects to the bones in the bottom of the foot. When it is damaged, it can result in flat feet. This type of occurrence is far more prevalent in women, especially those between 40 and 60 years old. It is believed to be the result of overuse of the posterior tibial tendon. It is also common among runners and athletes who play high-impact sports. Other causes include diabetes, obesity, and high blood pressure. Symptoms include pain on the inside of the foot and ankle, accompanied by swelling, redness, and warmth. Symptoms usually spike after engaging in physical activity, such as jumping and running. For more information on this type of injury, it is suggested that you make an appointment with a podiatrist.

Geriatric Foot Care
Hammertoes
Hammertoes get their name due to the abnormal appearance of the toe, in which it bends down like a hammer. A hammertoe’s shape is formed by a deformed bending in the toe’s middle joint. There are many factors that play into how hammertoes form. Trauma to the toe, such as stubbing, jamming, or breaking it, can make the toe more susceptible to developing into a hammertoe. Age and gender also influence the likelihood of getting a hammertoe. Women have also been known to be more likely to develop a hammertoe compared to men. You’re also more likely to develop this condition as you age. Certain diseases, such as arthritis and diabetes, have been known to influence the likelihood of forming a hammertoe.
Heel Spurs
A heel spur is a bony protrusion that occurs on the heel. It is often confused with plantar fasciitis, which is inflammation of the plantar fascia ligament. When a heel spur is painful it can make it quite difficult to walk.
Heel Pain
Heel pain is most commonly caused by repetitive stress. With that being said, it may be more common for athletes who perform repetitive movements to be afflicted with heel pain at some point during their activity. Heel pain can occur due to a number of different factors. These may include issues with having poor circulation, complications due to arthritis and diabetes, and poor form when running or exercising, as well as having poor posture.

Ingrown Toenails
Treatment for an ingrown toenail varies between patients and the severity of the condition. In most cases, it is best to see your podiatrist for thorough and proper treatment. After examining your toe, your podiatrist may prescribe oral antibiotics to clear the infection if one is present. Surgical removal of either a portion of the nail or the entire nail may also be considered. In some cases, complete removal or destruction of the nail root may be required. Most patients who undergo nail surgery experience minimal pain afterward and can return to normal activity the following day.

Plantar Fasciitis
Foot pain can significantly impact your quality of life, making it difficult to complete your daily activities. One common cause of foot pain is improper footwear, as wearing ill-fitting shoes or high heels can lead to foot disorders and discomfort. In addition, injuries sustained during high-impact exercises or sports activities, such as running or aerobics, may result in foot pain. This includes sprains, strains, fractures, and tears in ligaments, tendons, or muscles. Another cause of foot pain is osteoarthritis, which results from the overuse of cartilage surrounding the joints, particularly affecting the big toe. Rheumatoid arthritis also can lead to pain, stiffness, and swelling in the joints of the feet and ankles. For patients with diabetes, nerve damage, and reduced blood flow, foot ulcers may develop and cause persistent foot pain. Apart from these primary causes, various specific conditions can lead to foot pain, such as plantar fasciitis, corns, calluses, bunions, and ingrown toenails. Flat feet, heel spurs, tarsal tunnel syndrome, and Sever's disease are other causes of foot pain. If you are experiencing foot pain, it is suggested that you make an appointment with a podiatrist who can determine the cause and offer the appropriate treatment options.

Dr. Stiebel graduated from the University of Michigan, Ann Arbor Campus, and the Scholl College of Podiatric Medicine. He completed his preseptorship in conjunction with the Dearborn Medical Center Hospital. He is a member of both the Michigan and American Podiatric Medical Association, as well as a diplomat of the American Board of Podiatric Surgery and a fellow of the American College of Foot and Ankle Surgeons. He is affiliated with St. John Macomb Surgery Center. Dr. Stiebel has lectured on podiatric conditions in the United States and overseas. Dr. Stiebel participates in all aspects of podiatric medicine and performs podiatric surgeries. He especially enjoys evaluating foot mechanics and structure, yet is also motivated by challenging surgical cases. Dr. Stiebel has been treating patients in Macomb County since 1984.
