A: Most managed care programs require a written referral from your primary care physician. Without this form, your insurance company will refuse to pay for your visit, and therefore, you will be asked to pay for services at the time of your appointment, prior to being seen. We are required to comply with contractual rules of your health plan-so please help us by attending to your part of the paperwork.
Emergency Care
Neurosurgeons often use CT Scans to make urgent diagnoses and plan treatments for patients in the emergency room and at other times. "Teleradiology" is the technology that allows physicians to see CT Scans, MRI Scans and other imaging studies from a remote location. Our office computer is available to view such images from Caritas Norwood and Caritas Good Samaritan Hospitals. During off hours, this technology is also available to the "on call" physician. Soon after an emergency CT Scan has been completed and we are notified, the neurosurgeon can access the scan, review the images and advise the treating physicians and nurses at the hospital on the best treatments for these often critically ill patients. If surgery is indicated, the preparation of the patient and the operating room gets underway, even before the neurosurgeon arrives at the hospital. This time saving can make a big difference in a patient's outcome.
Electrocardiogram
A Peripheral Nerve Disorder is an abnormality of a nerve outside of the Central Nervous System (Brain and Spinal Cord). It is also called neuropathy. Peripheral nerves can be injured at any site but are most commonly injured by compression or trauma as they pass beside a bone or travel through a tunnel. Tests for Peripheral Nerve Disorders include an EMG (Electromyogram) and NCV (Nerve Conduction Velocities). These are electrical tests similar to EKG's done to evaluate the heart.

Carpal Tunnel Syndrome
An example of a compression neuropathy is Carpal Tunnel Syndrome. In this instance, the Median Nerve is compressed as it passes through a tunnel in the wrist. The symptoms often include numbness in the hand, electrical shocks from the wrist to the long finger, index finger or thumb, and weakness. The numbness can wake the patient from sleep and is often worse when the patient tries to use the hand at a level above the heart. Decompression of the carpal tunnel is usually curative. Another common compression neuropathy is the Ulnar Nerve at the elbow. The symptoms are similar to the Carpal Tunnel Syndrome but involve the ring and small fingers of the hand. Some people notice this as an over reactive "funny bone."

Depending on the diagnosis and problem, an operating microscope may be used to facilitate the surgery. The operating microscope provides additional light and magnification, making the surgery safer. Sometimes, a Neuro-navigation system is used to precisely localize a lesion in the brain. Neuro-navigation is a computerized system working with CT or MRI scans similar to the way GPS (Global Positioning System) works with roadmaps in automobiles.

Stereotactic surgery is minimally invasive surgery with extreme accuracy. It is the computerized link between a CT Scan or MRI Scan to the equipment used in the operating room. With a stereotactic technique, neurosurgeons can precisely place a needle into a very small lesion or tumor with an accuracy of one millimeter. Dr. Freed has been performing stereotactic minimally invasive brain surgery since its beginning in the early 1980's. Drs. Friedberg and Gieger had extensive training in this technology prior to joining the practice in July 1999 and December 2008 respectively.

Treatment for disc herniations can be conservative (non surgical) or sometimes requires surgery. Conservative treatments of the spine are physical therapy, stretches, exercises, Non Steroidal Anti-inflammatory medications, epidural steroid injections (injection of a steroid anti-inflammatory drug into the spine), etc. Spinal surgery must decompress (remove pressure) the nerve and / or spinal cord and must maintain stability of the bones in the spine. This sometimes requires a fusion (spinal stabilization using bone grafting). Sometimes application of metal devices to "lock" the bones together are used to augment the fusion.
