UNDER NEW MANAGEMENT!!! This company is now the epitome of disorganization. Their staffing pool is too small to adequately serve their current client base, let alone any new clients. You'll see a new face every week, and it is obvious that their caregivers walk in with no prior knowledge about what their role is in your home, so be prepared to explain your needs. When the scheduler can't find a caregiver for ONE shift, they terminate ALL services immediately, with no discussion, even though their client agreement states they are required to provide notice.

This week I assisted my friend to have her husband discharged from this rehab and nursing home, Oaklawn Rehabilitation. When we arrived, he was sitting in urine which had been there for hours. He asked me to hand him a urinal as it was located out of his reach as was his call light. The materials beneath him were soaked and smelled. He has lost significant weight since arriving to the rehab after open heart surgery (>30 lbs). During visits to the rehab, not one staff entered the room during the 3-4 hours we were there. This was every day. It is age discrimination since he is 82. Prior to his surgery, this man would cut the field with a tractor. I have been a nurse for 33 years and worked as a nurses aide in a nursing home for 3 years. He has fallen twice trying to go to the bathroom. I feel terribly about the other clients admitted to this home which is why I write. Who will help those without resources? I ask you... Dr. Suzette Scheuermann PhD RN
By far the most disgusting, unorganied facility i have eer encountered. I live in Minneapolis and my grandmother suffered a stroke last week and was discharged to this facility following her hospital stay. me, my mother nad my uncle flew from Minnesota to tend to my grandmother. My uncle and mother are my grandmothers power of attorney and upon arriving at th hosipital, they signed all of the neccary paperwork. The next day, a nurse came and asked for my mother to sign the same paperwork AGAIN because the staf lost it. my mom refused and informed them they already signed the fors and that they have copies. the staff insisted that they sign again.. my mom didnt. My grandmother has reported REPEATED unanswered call lights. They LOST her medication order from the doctor. They were supposed to install a heart monitor.. it was never installed. They gave her food on severeal occasions without providing water. CHOKING HAZARD!! Th food was under cooked and inedible. CONT>>>
I am being sued by Oaklawn because my father died before I could pay his bill.
Needs help with management no one seems to listen passes the BUCK so to SPEAK no one held accountable.
I stay here rehab after having my knee replace i love it staff was great except 3rd shift needs to be worked on and food needs some improving but would stay here again
Great Company! We used there services for our Uncle, 24/7 care, they were always on time, very friendly and priced within our family budget!! My Uncle ended up enjoying the company of there staff/ nurses so much that we saw a complete turn around in his mood within days!:) Thank you for EVERYTHING! When I first arrived, I noticed that there was no smell of urine, which is common for a nursing facility. The staff was welcoming and very accomodating. The food could use some improving; some meals are better than others.The facility is older, but is comfortable. It is difficult to have a loved one away from home, but I feel like my mother is safe while battling her illness. Visiting hours are flexible.
This is a wonderful community of delightful residents and caring staff. When my mom moves here, I know she will enjoy life at Elmcroft.
