Let me start by saying please do not bring your loved one to this place!! My Mom was there for about a month before we were able to move her. First time we tried to move her we were told we couldnt move her from one hospital to another?? She declined while being in their care. We were with her everyday until at least 7 pm every night. Nothing happened unless we did it ourselves or stayed on top of the staff. Not all staff was bad but majority was! One aide in particular saw my Mom after she hadn't seen her in a few days and said get her out of this place before she dies! Now what does that alone say? Her case manager Stephanie Fey was non responsive and did not serve any use. The last straw was went we went to visit early one morning and ahead her bedding was soaking wet! The room smelled strong of urine. Someone had pushed her breakfast tray over her lap and walked out. Wish I knew a way to make sure others won't suffer in their care! Food is bad for the most part too! Stay far away from here!!
The nurses and aides are great and do a great job. They really seem to enjoy helping people get better. Brandy and David were especially great on the 6th floor.
Dr. Baker is my doctor and I love him! I really like the aides I've had, especially Gary and Lisa. The nurses Bo and Cheryl always took great care of me. I really love them. Also, Todd is really sweet. They really explain things to me so that I fully understand it. In respiratory, I love Melissa and Annette. You can't forget Tim, he's my buddy. I love me some crazy Tim. George and Steph weren't my nurses but they take care of the lady beside me. They are so sweet and nice. It's hard to find people like that, they're so nice. They always come to my room with a smile and ask me how I'm doing. That's what people who are sick need, smiling faces. The manager is really nice up here. She said if I ever have any problems to let her know. She checks up on me all the time and I know she doesn't have to. Jackie is my baby, so sweet. Every time she here, she comes in with a smile. Lisa is a nice wound care nurse who knows her job. She will always joke around with me, she's really fun.
I didn't know what's expected and not expected but all the workers took great care of my wife. Everyone took the time to make sure that I understood my wife's illness and how long it would take for her to get better. They were just wonderful to both her and I.

This place exceeded our expectations when caring for our father. Great nursing care, great therapy. Everyone does a great job. One of the friendliness staff around. Everyone is always willing to help. Will Always Be Apart of Our Family.
not the place to be or go to rehab or live. I walked in for tour and placed a loved one here. Smells, staff not friendly. I just can not recommend this place at all.
I am a frequent visitor. 5his place has the happiest staff that provides great care. They know everyone that walks in the door. They are good at what they do. People go home after they get stronger they dont stay here forever too often but if they need to might as well stay at a place that treats you like family. The caregivers work hard here. You never smell anything bad in the hallways. Food looks great at meals. I would and have recommended this place
My mother was in Nazareth Home for rehab and we would never have her go anywhere else. Everyone in the facility treated my mother as if she were the only patient they had to care for. The food was excellent and mom never complained about anything. The facility is clean, bright and airy. The staff met with me, her caregiver, before they released her to make sure we were all on the same page as to her requirements. Thank you Nazareth Home for making a difficult situation more than bearable.
My mother was here for 4 years and my dad visited everyday. As a retired nurse I looked carefully for a good local home close to my dad and found it here ,.The building is old but they treated my parents like family and for that I will forever be grateful. I live in another state and this was very important to me.
