Kidney Stones
It is fairly common in children occurring in 3-6% of all children usually between the ages of 4 and 8 years. In children, it may be asymptomatic, or it may cause frank blood in the urine, voiding dysfunction, flank pain, abdominal pain, recurrent urinary attract infections or kidney stones.
Urinary Tract Infection
As we were finishing the exam, my nurse handed me the results of his urinalysis. It was normal except for a “large amount of blood” detected on the urine sample. His urine was clear and yellow which meant that he had “microscopic hematuria” meaning that the blood couldn’t be seen by the naked eye. I gave mom a container and asked her to drop off a first morning urine so I could double check the urine. Sure enough, the next morning his urine showed the same thing. The urine was clear yellow with no evidence of infection, protein, or glucose, but large blood was detected once again. I sent this urine to the lab for a urine culture to rule out a urinary tract infection (it was negative), and to measure the calcium and creatinine in the urine. The calcium/creatinine ratio (urine calcium level divided by the urine creatinine level) was in fact elevated at 0.42. The normal should be below 0.2 so this confirmed the diagnosis of hypercalciuria. I was intrigued as I had a similar patient just a few weeks earlier.

Multiple Sclerosis
Optic Neuritis is a condition that occurs when there is swelling, inflammation or irritation of the optic nerve which connects the eye to the brain and allows us to see. The exact cause of ON is uncertain, but it has been associated with a number of viral illnesses such as the flu, measles, mumps, Lyme disease, and cat scratch fever. ON is also associated with Multiple Sclerosis (MS) and may be the initial symptom of this disease. Fifty percent of patients with MS will develop ON.

Anxiety

Ultrasound
After making the diagnosis of hypercalciuria, I did a kidney ultrasound to make sure there were two normal kidneys with no evidence of kidney stones.
MRI
My patient was seen right away by the eye doctor who made the diagnosis in the office. His vision improved dramatically after a few days of steroids and we are awaiting approval for the MRI.
Computed Tomography
Talking to the distraught parent the first question would be how is he acting now? Does he seem stable and appropriate even though he may be understandably upset? What were the circumstances of the fall and what was he doing? Did he trip and fall forwards or backwards? Was he on a skateboard or did he fall from monkey bars? How far did he fall, and was it witnessed by anyone? Was there ANY loss of consciousness (LOC)? If there was even a second of LOC a trip to the ER for imaging such as a CT scan of the head would be necessary.

Fungus
Tinea versicolor is caused by the fungus Malassazia furor which lives on our skin and normally causes no problems. This fungus lives alongside our body’s skin cells. It helps fight off pathogens and exists in a sort of symbiotic relationship. But when certain conditions arise the fungus may grow out of control. Then this may affect the normal pigmentation of our skin. When this occurs it is known as Tinea versicolor.
