Lung Cancer
Leptomeningeal carcinomatosis (LC) occurs when cancer cells enter the cerebrospinal fluid and spread quickly throughout the nervous system. Once considered a rare complication of cancer or cancer relapse, the disorder is now more frequently identified since cancer patients are living longer and diagnostics have improved. Leptomeningeal carcinomatosis is a complication found in approximately five percent of cancer cases. Most often, it occurs in patients with cancers that spread through the central nervous system, including breast cancer, lung cancer and melanoma.

Neurology
Thoracic radiculopathy, occurring in the mid-back, causes pain that can be misleading to physicians. Thoracic nerves do not extend to the limbs, but instead, to the torso. Because of this, patients often report pain or numbness in the ribs, abdomen or side—which is similar to a gallbladder attack or shingles. The American Academy of Neurology describes it as “...burning, shooting or tingling radiating to one half of the chest wall. It can often be confused with pain from an abdominal or cardiac source.” [2] In rare, unfortunate cases, patients have surgery on their abdomen because a physician did not properly diagnose radiculopathy.
Carpal Tunnel Syndrome
Fibular tunnel syndrome (peroneal nerve compression). Though less common than carpal tunnel and ulnar nerve syndrome, fibular tunnel syndrome is potentially more devastating. Peroneal compression can lead to foot drop that may require ankle bracing. Other symptoms include lower leg and top-of-foot numbness.
Multiple Sclerosis
Irritation of the fifth cranial nerve (the trigeminal nerve) causes this type of neuralgia. Neuralgia may arise as part of the aging process, but in some cases, it is associated with another disorder like multiple sclerosis or similar conditions that damage the myelin sheath nerve covering. The pain disorder most often affects people over 50, but it can arise at any age. It is more common in women than men, though reasons for this are unknown. There is some evidence that trigeminal neuralgia may be inherited due to a genetic pattern of blood vessel formation.

Diagnosis of hydrocephalus begins with a complete physical and neurological exam to detect problems of brain function. Sophisticated imaging tests like MRI, CT scan and ultrasound (for infants) are then used to detect obstructions and determine hydrocephalus severity. On rare occasions when other tests fail to reveal the disease cause, a transducer is inserted to measure intracranial pressure.

After removing the appropriate portion of the skull, your PBSSM neurosurgeon uses an operating microscope and microsurgical techniques to safely and effectively remove the identified tumor or hematoma. The skull bone may be replaced immediately or after a few days to allow brain swelling to resolve. It is reattached using MRI-safe titanium plates and screws, and all tissue layers are sewn or stapled closed.

PBSSM specialties include minimally invasive surgery, radiosurgery and traditional surgery for brain tumors, brain cysts, chiari malformation, hydrocephalus, meningioma, neuralgia, pituitary tumors, subdural hematomas and other brain disorders. Diagnostic tests. We begin with x-rays, which enable us to rule out infections, tumors, etc. CT scans and MRIs offer three-dimensional views of the cervical spine and can help detect osteophytes (bony growths) and herniated discs. Your neurosurgeon may also suggest a myelogram, which involves injecting liquid contrast dye into the spinal column to reveal spinal cord pressure.
Chemotherapy
Leptomeningeal carcinomatosis’ increased incidence has led to more research into the composition of cerebrospinal fluid and LC tumors. With more knowledge of how the disease spreads in the nervous system, researchers hope to develop new, more effective treatments. In addition, early screening of patients at risk for the disease is improving detection before serious neurological damage occurs. With early intervention and stabilization, as well as more effective chemotherapy, there is hope for improved quality of life and increased chance of long-term survival.

Radiation therapy: This treatment involves using a machine to direct a beam of radiation or destructive energy at the tumor. The radiation damages the tumor, causing it to shrink or stop growing.

Occipital Neuralgia. With sudden, throbbing, electric-shock-like headache pain, this form of neuralgia results from irritation of the occipital nerves that pierce the muscles of the neck. It may be precipitated by whiplash, localized trauma, infection or chronic positioning of the head in a downward or forward direction.
Minimally Invasive Surgery
Kyphoplasty is a minimally invasive surgery performed under local or general anesthesia. The procedure rebuilds the vertebral space lost as a result of spinal compression fractures. First, a small incision is made in the skin of the back. Then, using advanced x-ray imaging technology, a narrow tube is guided through the skin to the fractured vertebra. A special balloon called a bone tamp is inserted through this tube and gently inflated, restoring the vertebral space to the desired height. The balloon is removed, and the restored space is filled with orthopedic cement called PMMA (Polymethylmethacrylate). As the cement hardens and repairs the fracture, it restores strength, stability and height to compressed vertebrae and relieves pain for the patient.
Joint Replacement
Osteoarthritis (OA) affects millions of people around the world and is particularly common in knees, hips, spine and hands. OA develops as the joints' protective cartilage wears down over time, causing loss of mobility and painful friction during joint movement. In worst cases, you may experience total cartilage loss, leaving the joint "bone on bone." This severe form of OA causes extreme pain and joint stiffness. Although osteoarthritis is degenerative, it can often be well-managed with conservative measures like prescribed activity recommendations, non-steroidal anti-inflammatories, chronic pain medications or physical and occupational therapy. Injections of corticosteroids or hyaluronic acid therapy may also be beneficial. If surgery is needed for joint replacement or bone realignment, Dr. Balar and his team will refer you to an expert surgeon in your area.

Plantar fasciitis is a common cause of heel and foot pain that develops when the plantar fascia, the soft tissue that connects toes to heel bone, becomes irritated or inflamed. This connective tissue disorder leads to stabbing foot pain that is particularly severe upon rising or moving after inactivity. Long periods of standing can also exacerbate plantar fasciitis. Runners and fitness enthusiasts are prone to developing plantar fasciitis, but being overweight also increases the risk for this condition. When plantar fasciitis is not diagnosed and treated correctly, it may lead to issues in the knee, hip and back.
