If you have an appointment for a Divorce, Child Custody, or Child Support, you are required to bring certain documents with you to your appointment.

If it is a new case, be sure that you have a Physician's Certificate of Medical Examination completed by a licensed Physician before your appointment. If you do not have a copy, please print the Certificate from the Forms tab drop-down menu. It must be less than 120 days old.
In order for Kara to properly draft the estate planning documents, the client must first complete what we call a Will Packet. This form simply details exactly how you want your estate to be handled.

Wills, Statutory Durable Power of Attorney, Medical Power of Attorney, Supported Decision Making Agreement, HIPPA Release, Directive to Physician, Declaration of Guardian, etc. Go to the Estate Planning tab under Forms to get the Will Packet.
