Pediatric Care
Sunil Nayak, MD performed his undergraduate studies at Duke University and graduated from the University of Virginia School of Medicine in 1990. He completed his pediatric internship and residency at Carolinas Medical Center in Charlotte, North Carolina, where he was Chief Resident. He completed his Pediatric Endocrine Fellowship training at the Children’s Hospital of Pittsburgh in 1997. Dr. Nayak joined Pediatric Endocrine Associates in 1997. He is Board Certified in Pediatrics and Pediatric Endocrinology.
Primary Care
Because constitutional delay is a variation of the growth process, your child will eventually enter puberty, followed by a growth spurt. To “jump start” this process, monthly testosterone injections may be given to boys over 6 to 12 months. These injections can be given at our office or through your primary care physician. The growth and development in the child will be carefully monitored at our office.

At Pediatric Endocrine Associates, P.C., we accept most insurance carriers. When you call to schedule an appointment, we are able to fit you in quickly. While our main office is located in Greenwood Village, we operate satellite locations, in Castle Rock, Colorado Springs, Louisville, Loveland, and downtown Denver. We do not charge facility fees when you visit our pediatric endocrinology offices.
Hypothyroidism
Congenital hypothyroidism is a condition in which children are born with an inability to make adequate amounts of thyroid hormones. Thyroid hormones are necessary for normal brain growth during the first 3 years of life. Therefore, proper treatment of children with congenital hypothyroidism must be started as soon as possible after birth in order to ensure normal brain growth and development.

Our Mission at Pediatric Endocrine Associates is to treat and care for infants, children, and adolescents in the Rocky Mountain region with endocrine (hormonal) disorders. This involves assisting the primary care physician in the assessment and management of children with conditions that include: diabetes, overweight, variations and disorders of growth and puberty, thyroid disorders, including thyroid cancer, adrenal disorders, disorders of sexual differentiation, calcium disorders/metabolic bone disease, hypoglycemia, and other endocrine-related problems. We are committed to providing excellent, evidenced-based care, in a caring and compassionate manner to assist our patients to lead normal lives.
Thyroid Cancer
Although radioactive iodine has been the preferred treatment in adults for a long time, pediatric endocrinologists used to be hesitant about using this treatment in children. This was because we did not have any long-term experience using radioactive iodine in children, and we worried that children might be prone to developing cancer in the remaining thyroid tissue. Now, after 30 years of using radioactive iodine in children, we have follow-up information from children treated at the Cleveland Clinic; those individuals who were treated as children with radioactive iodine for hyperthyroidism have had no increased occurrence of thyroid cancer. Similar follow-up information from Sweden also has confirmed that there is no increased occurrence of thyroid cancer in individuals treated with radioactive iodine therapy. Thus, our previous worries about the safety of radioactive iodine therapy in children seem to have been unfounded.

Ultrasound
Since there are many causes of precocious puberty, your doctor will ask many questions and perform a complete physical examination. Blood tests may be done to measure hormone levels that are increased in puberty. An X-ray of the left hand and wrist may also be taken to determine the bone age or the maturation of your child’s bones. Often a GnRH stimulation test may need to be done to determine whether the cause is central (the brain is signaling to start puberty). The test is 3 hours long. A medication is given, and then blood is drawn at hourly intervals. In girls a pelvic ultrasound is sometimes needed to check the adrenal glands, ovaries and uterus for abnormalities that may be responsible for causing early puberty. A MRI (magnetic resonance imaging) scan of the brain and pituitary gland may be done to look for abnormalities.
MRI
First the doctor will obtain a detailed history and a thorough physical examination. Blood tests will indicate hormone levels of pituitary hormones and target gland hormones. In some cases hormone stimulation tests are necessary to fully evaluate the functioning of the pituitary gland. A urine sample and electrolytes in the blood may be done, if ADH deficiency is suspected. An MRI (magnetic resonance imaging) can detect tumors or other structural abnormalities in the brain. An X-ray of the left hand and wrist will determine the child’s bone age, which shows the maturity of the child’s bones.

Nuclear Medicine
Using radioactive iodine, a nuclear medicine specialist destroys the overactive thyroid gland. First the child is treated with the anti-thyroid medication, then a test is done to determine the thyroid’s uptake of radioiodine and a scan is done to calculate the dose needed to destroy the thyroid gland. Finally, the radio-iodine is swallowed and the thyroid gland deteriorates.
Radiation Therapy
Hypopituitarism can be caused by anything that disrupts the normal functioning of the hypothalamus or the pituitary. Congenital hypopituitarism refers to the condition being present at birth, which means that either the pituitary or the hypothalamus did not develop normally prior to birth. Acquired hypopituitarism refers to damage to the pituitary or hypothalamus during or after birth. This can be caused by a severe head injury, surgical removal of the pituitary gland, radiation therapy, a brain tumor, or an inflammation or infection in the brain. In cases where the cause cannot be determined, it is often referred to as “idiopathic”, which means “of unknown cause”.
