Scleral buckle surgery has been widely used since the 1950s. In this procedure, a silicone band is placed around the outside of the eye to indent the wall of the eye toward the retina. This provides the retina a solid foundation to reattach to and relieves any potential traction on the retina. Scleral buckle surgery can be performed by itself or combined with a pars plana vitrectomy. When performed alone, cryotherapy is applied to the eye wall beneath the retinal tear to freeze the tissue. This freezing causes a scar to form to permanently adhere the retina to the wall of the eye and seal the retinal tear.
Primary Care
Blood Pressure Medication: For surgery to be done safely, blood pressure should be within normal range. If your body is used to blood pressure medication being taken in the morning, it is sometimes recommended to take the morning dose with a small amount of water on the day of surgery. Before surgery, it is essential to discuss with the surgery pre-operative screening and primary care doctor.

Dr. Patel grew up in St. Petersburg, Florida, where he graduated as valedictorian of his International Baccalaureate class at St. Petersburg High School. He attended the University of Pennsylvania, where he triple-majored in biochemistry, economics, and biophysics, graduating with Summa Cum Laude honors and a member of Phi Beta Kappa. Along with his undergraduate degree, Dr. Patel obtained a Master’s degree in Chemistry within his four years of study. He wrote his master’s thesis on original translational research that he performed on molecular drug development for gastrointestinal cancers. Dr. Patel jointly attended the Massachusetts Institute of Technology (MIT) and Harvard Medical School to pursue his medical studies as part of the Health Science and Technology (HST) program. Dr. Patel was awarded a Howard Hughes Medical Institute Medical (HHMI) medical research fellowship, performing translational research at Dana Farber Cancer Institute, focusing on developing new chemotherapies. During his clinical rotations, he discovered his passion for surgery and ophthalmology.
Macular Degeneration
Age-Related Macular Degeneration (AMD) is a degenerative condition involving the retina in the central area known as the macula. In most cases, the damage is mild, and the vision is normal, but the central vision can be lost entirely in advanced cases. Visual tasks that require detail are affected the most, making some daily activities very difficult. AMD does not lead to total blindness even as the disease progresses because peripheral vision remains unaffected. Retaining peripheral vision keeps patients independent even if the vision loss is severe.
Laser Eye Surgery
The most significant risk with any incisional surgery is the risk of infection; in the eye, this is known as endophthalmitis. Endophthalmitis is a severe condition that can lead to severe ocular damage and blindness. Great care is taken when performing eye surgery to minimize this risk, and this is why these procedures are performed under sterile conditions in the operating room. The eye is cleaned with betadine to kill surface bacteria, and antibiotics are used during and after surgery. New surgical techniques with small incisions have significantly decreased the risk of infection.
Cataracts
While the process is a natural part of eye aging, specific conditions can accelerate the process. Patients with high myopia who have had trauma to the eye, undergone cataract extraction or other intraocular surgery, and have had YAG capsulotomy are at risk of having an acute posterior vitreous detachment. When the vitreous detaches secondary to one of these causes, there is an increased risk of developing a retinal tear.

Diabetic Retinopathy
Diabetes and elevated blood glucose levels cause blood vessel damage throughout the body. The damage is most severe to small blood vessels, especially the eye, kidney, fingers and toes, heart, and brain. As long as the blood glucose is elevated, microscopic damage to the cells that line the walls of small blood vessels and microaneurysms form. These are focal areas of weakness in the walls of the blood vessels that can then bleed or leak fluid and lipid into the retina as the earliest signs of diabetic retinopathy.

Ultrasound
Essential for diagnosis is a thorough examination of the eye and medical history. The pattern of inflammation seen dramatically narrows down the numerous causes of uveitis. If the posterior structures of the eye are involved, a Fluorescein Angiogram is performed to evaluate the optic nerve and retinal blood vessels for signs of inflammation. Often an Optical Coherence Tomography (OCT) scan is performed to evaluate and measure any retina swelling. A B-scan ultrasound may be used to assess any inflammation and fluid in the linings of the eye. Based on the exam findings, laboratory workup is undertaken to look for infection signs or an underlying autoimmune disorder. These lab tests are helpful but commonly do not result in a definite diagnosis. They do, however, eliminate particular possibilities that are the most serious.

In these cases, the most those providers may bill you is your plan’s in-network cost-sharing amount. This applies to emergency medicine, anesthesia, pathology, radiology, laboratory, neonatology, assistant surgeon, hospitalist, or intensivist services. These providers can’t balance bill you and may not ask you to give up your protections not to be balance billed.
